The management of unruptured and ruptured brain arteriovenous malformations (AVMs) remains controversial. The Treatment of Brain AVM Study (TOBAS) was designed to assess curative treatments in the management of AVMs. The purpose of our study is to provide a care trial context to brain AVM patients.
TOBAS is a pragmatic, prospective study including 2 randomized controlled trials and a registry. All AVM patients can be recruited. The preferred management modality will be predetermined prior to randomization by the team based on clinical judgment. Patients eligible for both conservative and interventional management will be randomly allocated conservative or curative treatment. Randomization will be stratified by a treatment modality (surgery, radiosurgery or embolization) and minimized according to a history of previous rupture and Spetzler-Martin grade. A second randomization will allocate eligible patients to embolization/no embolization prior to surgery or radiosurgery. The primary outcome of the study is death (any cause) or disabling stroke (mRS > 2) at 10 years. All patients managed according to clinical judgment alone will be included in the registry. A minimum recruitment of 540 patients is required to show that treatment can reduce the primary outcome by 10 % (from 25 to 15 %); 440 patients will be needed to show a 10 % increase in angiographic occlusion for a good clinical outcome with pre-embolization.
The trial is designed to offer optimal and verifiable care to patients with brain AVMs in spite of the uncertainty. We are currently seeking the participation of multiple centers.
La prise en charge des malformations artérioveineuses cérébrales (MAVs) rompues et non rompues demeure controversée. L’étude TOBAS (Treatment of Brain AVM Study) a été conçue pour évaluer le rôle des traitements curatifs dans la prise en charge des MAVs. Notre objectif est de proposer une étude dans un contexte de soins chez les patients porteurs d’une MAV.
TOBAS est une étude pragmatique, prospective, incluant 2 essais randomisés contrôlés associés à un registre. Elle vise à inclure tous les patients porteurs d’une MAV. La modalité du traitement curatif est prédéterminée par l’équipe traitante avant la randomisation en fonction du jugement clinique. Les patients pour lesquels un traitement conservateur et un traitement curatif sont envisageables sont randomisés. La randomisation est stratifiée selon la modalité de traitement (chirurgie, embolisation, radiochirurgie) et minimisée selon le grade de Spetzler-Martin et la présentation, hémorragique ou non. Le rôle de la pré-embolisation est évalué lors d’une deuxième randomisation chez les patients éligibles à une chirurgie ou une radiochirurgie. Le critère de jugement principal est la survenue d’un décès ou d’un accident vasculaire cérébral débilitant (mRS > 2) à 10 ans. Tous les patients non randomisés sont inclus dans le registre. L’inclusion de 540 patients est nécessaire pour montrer qu’un traitement peut diminuer les risques de décès ou d’accidents débilitants de 10 % ; 440 patients doivent être inclus pour valider l’hypothèse que l’embolisation pré-chirurgicale ou pré-radiochirurgicale augmente de 10 % (80 à 90 %) les chances de cure sans complication invalidante.
TOBAS a pour but d’offrir un contexte d’étude clinique approprié à l’incertitude liée aux traitements des patients porteurs d’une MAV. Nous sollicitons activement la participation d’autres centres.
