2021, Radiotherapy and OncologyCitation Excerpt :Total body irradiation has been established as part of the conditioning for HSCT at The Royal Marsden Hospital since the 1970s. Barrett et al. [22] described the incidence of both infective and idiopathic IP in patients with acute leukaemia, from 1974 onwards, treated with a low dose rate cobalt-based TBI technique; 5% of HSCT patients in this series died after developing IP. Following these early experiences with TBI there has been continued efforts to harness new technologies to minimise the lung toxicity in transplant patients, including the installation of a double-headed cobalt-60 unit for large field radiotherapy in the 1980s [23].

A relationship between pulmonary toxicity (PT), and pretransplantation total body irradiation (TBI) conditioning has been suggested but remains controversial (4, 9-13). As a result, a consensus regarding an optimal radiation dose schedule, as well as TBI technique and dose constraints to the lungs, is lacking.
