The antigen/receptor specificity of antigranulocyte antibodies in patients with SLE

Abstract

The antigen/receptor specificity of antigranulocyte antibodies (AGAs) detected in the sera of patients with systemic lupus erythematosus (SLE) was investigated by inhibitory immunofluorescence test and Western immunoblotting technique.

The interactions of AGAs with antigens of intact normal granulocytes were determined by inhibiting the binding of different myeloid monoclonal antibodies (mAbs). Seven of the studied 12 sera revealed binding to CD15 (X hapten) and/or to CD16 (FcR_lo).

The specificity investigation of AGAs was completed with Western immunoblotting technique. The binding of AGAs to bands with M_r of about 50–60 kDa and at 30 kDa on unstimulated granulocyte plasma membrane preparation could be demonstrated from 4 out of 6 AGA positive SLE sera. The cause of the disappearance of bands on the phorbol-myristate-acetate (PMA) activated membrane except those of the 50–60 kDa bands is still to be discovered.

Cette revue générale concerne l'ensemble des données publiées sur les autoanticorps antirécepteurs et autoanticorps anticanaux transmembranaires. La nature des cibles reconnues par ces autoanticorps, les pathologies associées, leur pouvoir pathogène et les principales techniques de détection utilisées pour les mettre en évidence sont présentées. Parmi les autoanticorps décrits, certains sont directement impliqués dans la pathogénie des maladies auto-immunes, pour d'autres, leur rôle pathogène n'est pas certain. Même si l'intervention de ces autoanticorps dans le développement des maladies auto-immunes est limitée, leur présence est importante sur le plan du diagnostic avec des marqueurs très spécifiques de certaines pathologies ; ainsi, certains autoanticorps sont recherchés en pratique courante comme les autoanticorps antirécepteurs de l'acétylcholine (RACh), les autoanticorps anticanaux calciques et potassiques ; d'autres autoanticorps ne sont recherchés que ponctuellement et uniquement par des laboratoires spécialisés. Cette revue sera divisée en trois parties, nous permettant de décrire l'ensemble des principaux autoanticorps antirécepteurs et anticanaux décrits ; cette liste n'est cependant pas exhaustive. La famille des autoanticorps antirécepteurs membranaires dont les autoanticorps antirécepteurs à activité enzymatique (souvent tyrosine kinase) associée ou intrinsèque avec un seul domaine transmembranaire, les autoanticorps antirécepteurs liés aux protéines G avec sept domaines transmembranaires, les autoanticorps anticanaux ioniques et les autoanticorps antirécepteurs liés à la membrane par le glycosylphosphatidylinositol puis la famille des autoanticorps antirécepteurs intracellulaires dont les autoanticorps antirécepteurs des estrogènes, les autoanticorps antirécepteurs des androgènes, les autoanticorps antirécepteurs de la lamine et les autoanticorps antirécepteurs de la kinésine seront décrits.
This review of literature concerns the different autoantibodies directed against membrane receptors and ion channels. The target antigens, the associated pathologies, the pathogenesis and the methods of detection of these autoantibodies will be addressed. Some of these autoantibodies are thought to be closely related to the auto-immune disease whereas for some others their pathogenesis role is still unclear. Overall, the roles of antibodies are different between diseases, but the presence of such autoantibodies support the basis of intervening immunotherapy, antibody titers predicted the activity of the diseases and some of them are very specific and become the useful markers for the diagnosis. Some autoantibodies are detected routinely as the antiacetylcholine receptor, voltage-gated potassium and calcium channels autoantibodies whereas most of them are detected very rarely and only by specialized laboratories. This review will be divided in three parts with the following classification: the first group of autoantibodies directed against membrane receptors included receptors with an enzymatic activity (mostly tyrosine kinase) with one transmembrane domain, receptors associated to G protein with seven transmembrane domains, ion channels and receptors associated to the membrane by the glycosylphosphatidylinositol and the second group of intracellular receptor autoantibodies directed to the estrogens, androgens, lamin and kinesin receptors.
An Fc<inf>γ</inf>RIII (CD16)-specific autoantibody from a patient with progressive systemic sclerosis

Polyspecific and organ specific autoimmune diseases are often accompanied by prolonged clearance of immune complexes. In mice, impaired macrophage Fc_γ receptor function may be associated with autoantibody against Fc_γ receptors. To extend these observations to autoimmune human disease, we transformed with EBV peripheral lymphocytes from a patient with terminal progressive systemic sclerosis and screened for clones secreting anti-Fc_γ receptor Ig. A clone, N55, which secretes a high affinity anti-Fc_γ receptor IgG₂ antibody was obtained. The Fab fragment of N55 bound to human neutrophils, NK cells, but not to monocytes, consistent with specificity for Fc_γRIII (CD16). N55 Fab competed weakly for the binding of anti-Fc_γRIII mAb 3G8 to neutrophils but did not have any effect on staining with the anti-Fc_γRII mAb, IV.3. N55 Fab did not bind to peripheral monocytes, but did bind to monocytes incubated with TGF-β (24 h) to induce Fc_γRIII. The specificity of N55 IgG for Fc_γRIII was confirmed by ELISA using secreted recombinant Fc_γRIIA and Fc_γRIIIB protein to coat microtiter wells. N55 IgG triggered the release from neutrophils of β-glucuronidase, arylsulfatase and alkaline phosphatase. Such antibody may play a pathogenic role in progressive systemic sclerosis.
