Abstract

A case report of the management of an ameloblastic fibroma causing the impaction of a mandibular first molar, with subsequent orthodontic care to salvage the impacted mandibular first molar, is presented.

The tumors reported as “granular”, “cystic” or “periphery” AF, or diagnosed as AF but having dental hard tissue formation10–15 were excluded from this study. The cases reported by Nilsen and Magnusson16 and Baroni et al.17 were also reported in other articles18,19 and they were only recorded once. Thus, a total of 123 cases were selected and their clinical data were summarized in Table 1.4,6–10,18–67
To further elucidate confusions with respect to the nature and biological behavior of ameloblastic fibroma (AF), available English-language literature since its first description in 1891 was reviewed. A total number of 123 cases with well-documented follow-up data were retrieved to evaluate various clinical, pathological and behavioral aspects of this tumor. AF tends to occur in the first two decades (89/123; 72.4%), but patients older than 22 years are not uncommon (30 cases). An overall recurrence rate of 33.3% (41 cases) is identified in reported cases who were treated by conservative (91.5%) and radical (8.5%) methods. Malignant transformation is evident in 14 recurrent tumors with an overall transformation rate of 11.4%. These data support the view that majority of AFs are true neoplasms. However, a small number of AFs occurring in childhood may represent the primitive stage of a developing odontoma, as three of the reported recurrent AFs do show further maturation with formation of dental hard tissues. A significantly longer recurrence-free survival was noted in patients treated by radical procedures in comparison to those treated by conservative methods and the age of patients at the first presentation was significantly related to malignant transformation of AF. As we are unable, at present, to differentiate a hamartomatous lesion from a neoplasm among this group of lesions merely on histologic grounds, age of the patients should be an important consideration when choosing therapeutic methods. Radical surgery should not be employed for the treatment of AFs in young people.
Mixed odontogenic tumours and odontomas. Considerations on interrelationship. Review of the literature and presentation of 134 new cases of odontomas

Based on a world-wide literature survey of published cases of “mixed odontogenic tumours” (ameloblastic fibroma, fibrodentinoma and fibro-odontoma) and complex/compound odontomas (including 134 own cases of odontomas) the authors present data showing the complex nature of these lesions. The authors suggest the following work hypothesis regarding the pathogenesis and relationship between the “mixed odontogenic tumours” and the odontomas. The tumours develop along two separate lines: (I) the neoplastic line comprising only one tumour, the ameloblastic fibroma (AF) and the closely related ameloblastic fibrodentinoma (AFD). (II) The hamartomatous (or the developing complex odontoma (DCO) line comprising: (1) The AF (and AFD). Differences in age and biological behaviour indicate that some AF are true benign neoplasms, whereas others are hamartomas presenting the first stage in the DCO-line. (2) The AF-O represents the second stage of the DCO-line developing into (3) the fully mineralized complex odontoma. Lastly, the authors suggest that the compound odontoma should be considered not as an alternative final stage to the complex odontoma but rather as a malformation (with a high degree of histomorphological differentiation) pathogenetically closely related to the process producing hyperodontia, “multiple schizodontia” or locally conditioned hyperactivity of the dental lamina.
