Another possible explanation is that caffeine, which has a diuretic effect in the blood stream, reduces therapeutic lithium levels (Jefferson, 1988; Mester et al., 1995). Thus, although the consumption of Red Bull precipitated the sudden onset of acute mania in the case study of Machado-Vieira et al. (2001), it is not clear whether psychoactive ingredients directly triggered the manic episode or whether caffeine may have increased the excretion of lithium, which in turn precipitated the manic episode.
