      Patients who received the combination of vascular endothelial growth factor and l-arginine had improved anterior wall perfusion on positron emission tomography (P = 0.02), a trend toward smaller perfusion defects (P = 0.10), and better anterior wall contractility (P = 0.02, Kruskal–Wallis) at 3 months versus baseline. However, dietary supplementation of l-arginine has been shown to reverse the inhibition of angiogenesis that results from endothelial dysfunction in many animal experiments (Voisine et al., 2005a, 2005b; Lerman et al., 1998) and also improve coronary endothelial dysfunction in humans (Tousoulis et al., 1998; Lekakis et al., 2002; Koifman et al., 2006), while EMAT is the first study to examine whether co-intervention of endothelial repair by oral supplementation of l-arginine can enhance pro-angiogenic efficacy. Indeed, data from EMAT are hypothesis-generating to help explain why therapeutic angiogenesis have so far largely failed in patients.
