Riassunto
Premesse.
L’inappropriatezza dei test diagnostici è un problema reale in Medicina di Laboratorio con forti risvolti di tipo etico, di malasanità, di risk management e di costi. Il Laboratorio di analisi non deve essere considerato semplicemente un servizio a disposizione dei reparti, un centro di produzione di numeri. Il ruolo di un moderno Laboratorio clinico è cambiato e gli esami di laboratorio richiesti in modo appropriato adesso contribuiscono in maniera considerevole al processo decisionale clinico.
Metodi.
Abbiamo valutato per un periodo di un anno le prescrizioni dei test diagnostici di laboratorio riguardanti la funzionalità prostatica. Riconoscendo la forte inappropriatezza della richiesta del PSA libero, ci siamo attivati con un piano operativo per una più attenta appropriatezza prescrittiva implementando l’avvio di un PSA riflesso.
Risultati.
I risultati di questa implementazione hanno ridotto drasticamente l’inappropriatezza prescrittiva del PSA libero, raggiungendo un lusinghiero 92,7% di appropriatezza nella richiesta del PSA libero contro il precedente 36,6%.
Conclusioni.
L’appropriatezza prescrittiva è un motore da spingere al massimo per i risultati che può dare in termini di miglioramento delle cure mediche e di contenimento e riduzione dei costi.
Summary
Background.
The inappropriateness of diagnostic tests is a real problem in Laboratory medicine with strong implications of ethical, medical malpractice, risk management and cost. The Laboratory analysis should not be considered simply a service available to departments, a center for the production of numbers. The role of a modern clinical Laboratory has changed and the laboratory tests required appropriately now contribute significantly to the clinical decision-making.
Methods.
We evaluated for a period of one year the requirements of laboratory diagnostic tests regarding functionality prostatic. Recognizing the strong request of the inappropriateness of free PSA, we went into action with an operational plan for a more careful prescription appropriateness implementing the start of a PSA reflex.
Results.
The results of this implementation have dramatically reduced the prescription unsuitability of free PSA reaching a gratifying 92.7% of appropriateness in the application of the free PSA against the previous 36.6%.
Conclusions.
The appropriateness of the request is an engine to push hard for results that can give in terms of improvement of medical care and in terms of economic savings could guide the spending towards containment and cost reduction.
