Inleiding
De C-reactiefproteïne(CRP-sneltest en goede consultvaardigheden helpen huisartsen bij het selecteren van de juiste patiënt met acute hoest voor behandeling met antibiotica. Daarmee kunnen ze onnodig antibioticumgebruik voorkomen. De CRP-test heeft een plaats gekregen in de NHG-Standaarden Acuut hoesten en Diverticulitis.
Achtergrond
Stichting Gezondheidscentra Eindhoven (SGE) en het eerstelijnslaboratorium Diagnostiek voor U hebben gezamenlijk een kwaliteitssysteem ontwikkeld met als doel de CRP-sneltest goed en naar tevredenheid van de gebruikers in te zetten in de dagelijkse huisartsenpraktijk.
Belangrijkste kwaliteitsmaten
We beschrijven de ervaringen met de opzet van een kwaliteitsborgingssysteem en de mate van tevredenheid van patiënten, huisartsen en praktijkassistentes over de inzet van de CRP-sneltest in de huisartsenpraktijk.
Interventie
Vóór plaatsing van de gevalideerde testapparatuur volgden alle 46 huisartsen van de SGE de NHG-geaccrediteerde nascholing opwww.acutehoest.nl. Praktijkassistentes kregen een training in de praktijk. Een labmedewerker controleerde maandelijks op naleving van de werkprotocollen en verrichtte controlemetingen van de testapparatuur. We hebben een circuit voor elektronische data-uitwisseling ingericht. Het laboratorium declareerde de CRP-sneltest bij de zorgverzekeraar.
Effecten
In 2,5 maand hebben praktijkassistentes bij 936 patiënten CRP-sneltests verricht. De analytische kwaliteit van de testapparatuur bleef geborgd. Het gebruik van een internetinterface als koppeling tussen Huisarts Informatie Systeem en Laboratorium Informatie Systeem verliep niet zonder problemen, maar die bleken grotendeels op te lossen. Patiënten, huisartsen en praktijkassistentes waren (zeer) tevreden over het consult, de CRP-testprocedure en de bijdrage van de CRP-sneltest aan de kwaliteit van de geleverde zorg aan de patiënt.
Leerpunten en vervolg
Samenwerking tussen huisartsenpraktijken en een geaccrediteerd (eerstelijns)laboratorium bleek in dit implementatieproject succesvol en maakte een hoogwaardig CRP-sneltestgebruik in de huisartsenpraktijk mogelijk. Hiermee is een aanzet gegeven voor toekomstbestendigepoint of care testing in de eerste lijn. ICT-ontwikkelingen en goede financieringsafspraken met de zorgverzekeraar zijn nodig om deze kwaliteitsimpuls voor elke patiënt mogelijk te maken.

Introduction
Point-of-care testing for C-reactive protein (CRP) and good communication skills help general practitioners to select appropriate patients with acute cough for treatment with antibiotics. In this way, extended use of antibiotics can be prevented. The point-of-care CRP test has been included in the Dutch College of General Practitioners Guidelines on Acute Cough and Diverticulitis.
Background
Stichting Gezondheidscentra Eindhoven (SGE: Healthcare Centres Eindhoven) and Diagnostiek voor U, a primary care diagnostic laboratory, have together developed a quality control system to ensure the quality of, and satisfaction with, the point-of-care CRP test for use in general practice.
Main outcome measures
Development of a quality control system, and patient, GP, and GP assistant satisfaction with the point-of-care CRP test in a general practice setting.
Intervention
Before being issued with CRP test devices, all 46 GPs of the SGE completed the course in the management of acute cough (www.acutehoest.nl) accredited by the Dutch College of General Practitioners. GP assistants received training in the practice. Each month, a laboratory assistant monitored the use of protocols and checked the accuracy of the CRP test device. A system for electronic data exchange was set up. The laboratory costs of the CRP test were covered by health insurance.
Results
In 2.5 months, CRP tests were performed in 936 patients. The analytical quality of the CRP device was sound. Use of the Internet interface that coupled the GP Information system with the Laboratory Information System was not without problems, but these were not insurmountable. Patients, GPs, and GP assistants were satisfied to very satisfied with the consultation, CRP test procedure, and contribution of the point-of-care CRP test to the quality of patient care.
Conclusions
The collaboration between GP practices and a primary care diagnostic laboratory proved to be successful in this implementation study, which opens the way to the introduction of point-of-care CRP testing in general practice. Further developments in information technology and good financial agreements with health insurance companies about costs are needed to ensure that this step towards improving patient management and care is available to all patients.
