Abstract Cognitive behavioural therapy (CBT) in people with an at risk mental state for psychosis is a relatively new treatment approach meant to prevent or delay transition to psychosis and/or schizophrenia. The theory behind this treatment is reviewed and illustrated by a case report. CBT in this population is a combination of three parts: in the first place ordinary CBT comparable with CBT for anxiety disorders, in the second place metacognitive training and in the third place normalizing psycho-education to stimulate the formation of alternative instead of catastrophic interpretations in order to reduce anxiety.
