Zusammenfassung

Endodontisch versorgte Zähne mit und ohne Wurzelspitzenresektion bedeuten im Kindesalter über die Zeit häufig eine Odyssee endloser Therapieversuche von geringer Nachhaltigkeit. Nicht selten münden wiederholte zahnerhaltende Maßnahmen in einer protrahierten Inflammation und enden letztlich mit dem Verlust des Zahnes. Angesichts der vielen Misserfolge, die sich auch aus der Literatur prominenter Arbeitsgruppen ergeben, sollten die seit Jahrzehnten bewährten Konzepte Wurzelkanalbehandlung und Wurzelspitzenresektion (WSR) als Standardtherapie bei Kindern und Jugendlichen kritisch hinterfragt werden. Aus heutiger Sicht steht als langfristige Therapie insbesondere im Wachstumsalter der kieferorthopädische Lückenschluss als einzige Alternative mit allen Vorteilen einer geschlossenen natürlichen Zahnreihe zur Verfügung.

Abstract

Teeth subjected to endodontic treatment with or without apicectomy in childhood are frequently associated with an odyssey of endless treatment attempts of little sustainability. Quite often repetitive conservative measures culminate in protracted inflammation and eventual loss of the tooth. In view of the numerous failures reported by prominent study groups in the published literature, the concepts of root canal treatment and apicectomy which have been successfully used for several decades as a standard therapy for children and adolescents should be subjected to a critical assessment. Currently, orthodontic space closure is the only alternative long-term therapy, especially in adolescents, because it provides all the advantages of naturally closed dental arches.
