Résumé

Les lymphomes hépatiques correspondent le plus souvent à des localisations secondaires de lymphomes hodgkiniens ou non hodgkiniens ganglionnaires ou extraganglionnaires, de type B ou T selon la classification OMS. Ainsi, l’atteinte hépatique « secondaire » définit le stade IV de la classification d’Ann Arbor. La symptomatologie clinique est variée. Une cytolyse et/ou une cholestase peuvent être notées. La biopsie hépatique avec examen immunohistochimique confirme le diagnostic. Le traitement est basé sur une polychimiothérapie. Nous rapportons l’observation d’un patient ayant une hépatite chronique virale B et une infiltration hépatique par un lymphome non hodgkinien de phénotype B découvertes dans l’exploration d’une ascite récente.

Abstract

Hepatic localization of non-Hodgkin lymphoma is generally secondary, B or T-cell lymphoma, according to the classification by the World Health Organization. The liver involvement in lymphoma corresponds to stage IV of the Ann Arbor staging classification. The symptomatology is varied. Cytolysis and/or cholestasis can occur. Liver biopsy associated with an immunohistochemistry study confirms the diagnosis. Treatment consists of a polychemotherapy. We report the case of a patient presenting an ascites, which revealed a non-Hodgkin lymphoma and chronic hepatitis B.
