What is the best treatment for peritonitis secondary to diverticulitis: Hartmann’s procedure or resection with anastomosis?

Résumé

La réalisation d’une résection-anastomose colique (avec ou sans stomie de protection) et l’intervention de Hartmann représentent deux traitements proposés en urgence pour la prise en charge des péritonites d’origine diverticulaire. L’analyse de la littérature souligne l’absence d’étude de forte puissance comparant ces deux attitudes et ne permet pas de donner des recommandations sur le choix de l’intervention à réaliser avec un haut niveau de preuve.

Abstract

Primary resection with anastomosis (with or without a diverting stoma) and Hartmann’s procedure are 2 treatment methods used in emergency surgery for peritonitis secondary to acute colonic diverticulitis. A systematic review of the literature comparing the 2 procedures has been carried out. It highlights a considerable degree of selective bias that limits high-grade recommendations concerning the most appropriate procedure in such cases.
