Résumé

La chirurgie du sein peut entraîner deux principaux types de douleurs séquellaires chroniques, le syndrome douloureux post-mastectomie (ou syndrome douloureux postdissection axillaire) et les douleurs de sein fantôme. D’évolution lente et de résolution difficile, ces syndromes douloureux neuropathiques requièrent une attention toute particulière chez des femmes devant déjà gérer un contexte carcinologique. Entre 16 000 et 25 000 patientes françaises sont concernées, chaque année, par ces syndromes douloureux séquellaires. La prise en charge doit être la plus précoce possible et fait, la plupart du temps, appel à une organisation interdisciplinaire mettant en oeuvre des thérapeutiques médicamenteuses, physiques, sociales et psychologiques. La prévention primaire reste difficile, et le dépistage précoce éviterait un nomadisme médical et des délais de prise en charge encore trop tardifs.

Abstract

Breast surgery can entail two types of sequelar pains: post-mastectomy pain syndrome (PMPS or postaxillary dissection pain syndrome) and phantom breast pain. Slowly evolving and difficult to resolve, these neuropathic pain syndromes require particular attention in women who have to deal with a carcinological context. Between 16,000 and 25,000 patients are affected, each year, by these sequelar pains syndromes in France. Treatment should start as early as possible and, most of the time, requires multidisciplinary collaboration combining medicinal, physical, social and psychological therapies. Prevention remains difficult and early screening should obviate medical nomadism and ensure an earlier start to treatment.
