Zusammenfassung
Bei der Hämodialysebehandlung bestehen technische und pathophysiologisch begründete Patientenrisiken. Die im klinischen Alltag wichtigste und durch immer noch unzureichende technische Überwachungsmöglichkeiten vermittelte Gefahr liegt im unbemerkten venösen Nadelverlust und/oder in der Dekonnektion im venösen Schenkel. Diese Komplikationen führen zu einer unbekannten Anzahl von Todesfällen; geschätzt wird weltweit ein 3‑stelliger Bereich jährlich. Eine verbesserte Patientenüberwachung, sinnvolles Druckmonitoring, Nadelfixierung und evtl. neue Überwachungstechniken können diese erhebliche Gefahr minimieren. Bei der regionalen Zitratantikoagulation (rCA) müssen manuelle Überwachungen die ebenfalls seitens der Industrie noch nicht entwickelten Verbindungsschnittstellen der Zitrat- und Kalziuminfusion ersetzen, um schwerwiegende metabolische Probleme, auch mit möglicher Todesfolge, zu verhindern. Pathophysiologische Dialyserisiken wie plötzliche schwere Bewusstlosigkeiten mit Organminderperfusionen können und sollten vermehrt durch eine physiologische Dialysesteuerung verhindert werden.
Abstract
Hemodialysis treatment is associated with technical and pathophysiologically based risks for patients. The most important risk in the clinical practice, mediated by still inadequate technical monitoring capabilities, is unnoticed venous needle loss and/or disconnection in the venous limb. These complications lead to an unknown number of deaths, estimated annually to be in the range of three digits worldwide. Improved patient monitoring, meaningful pressure monitoring, needle fixation and possibly new monitoring techniques can minimize this significant risk. In regional citrate anticoagulation (rCA), manual monitoring must replace the not yet commercially developed connection interfaces of citrate and calcium infusion to prevent severe metabolic problems, including possible death. Pathophysiological dialysis risks, such as sudden severe unconsciousness with diminished organ perfusion can and should be increasingly prevented by physiological dialysis control.
