Zusammenfassung
Patienten mit chronisch-entzündlichen Darmerkrankungen (CED) haben ein erhöhtes Risiko für die Entwicklung von Kolonkarzinomen. Risikofaktoren sind unter anderem die chronisch-persistierende Entzündungsaktivität und bei der Colitis ulcerosa die Krankheitsausdehnung. Die frühzeitige Detektion von Dysplasien und Frühmalignomen ist durch die Etablierung von entsprechenden hochqualitativen Vorsorgeendoskopien, z. B. mit Chromendoskopie, möglich geworden. Durch eine rechtzeitige risikoadaptierte endoskopische Überwachung kann die Prognose der Patienten deutlich verbessert werden.
Abstract
Patients with inflammatory bowel disease (IBD) have an increased risk of developing colorectal cancer. Risk factors include, among others, chronic inflammation and extensive ulcerative colitis. The prognosis of colorectal cancer is dependent on early detection of premalignant lesions. Therefore, endoscopic surveillance for patients with IBD has been shown to be highly effective to reduce the incidence of colorectal cancer. In clinical routine, tight control and high-quality endoscopic surveillance, e.g., chromoendoscopy, for patients with IBD in accordance with individual prognostic risk factors are highly recommended.

Es sind keine regelmäßigen jährlichen Überwachungsmaßnahmen nötig.
Aufgrund der Einnahme von Azathioprin sollte er alle 2–3 Jahre koloskopiert werden.
Die Behandlung mit Azathioprin sollte sofort beendet und durch Mesalazin ersetzt werden.
Aufgrund der effektiven Behandlung mit Azathioprin muss er nie wieder koloskopiert zu werden.
