Quetiapine in the treatment of mania in a naturalistic setting: Results of an open, multicenter study

Zusammenfassung

Mehrere randomisierte, kontrollierte Studien (RCTs) belegen die Wirksamkeit von Quetiapin in der Therapie manischer Episoden bei bipolarer Erkrankung. Die Situation in klinischen Prüfungen unterscheidet sich jedoch in wesentlichen Punkten von der Anwendung in der täglichen Praxis, in der die Patienten nicht streng selektioniert und evaluiert werden. Ein Großteil der Patienten, die tatsächlich wegen akuter Manie in Behandlung stehen, erfüllt zumindest ein oder mehrere Ausschlusskriterien zur Teilnahme an RCT's, viele Patienten verweigern die Teilnahme an derartigen Studien wegen der durch das Studiendesign eingeschränkten Behandlungsoptionen, oft auch aus krankheitsbedingten Gründen, weswegen die Ergebnisse aus RCT's nicht vollständig auf die klinische Praxis übertragen werden. Durch naturalistische, nicht randomisierte Untersuchungen werden zusätzliche wertvolle Informationen unter Bedingungen, die der klinischen Routine eher entsprechen, gewonnen. Ziel der vorliegenden Untersuchung war es, die Wirksamkeit und Verträglichkeit von Quetiapin an einer großen Zahl unselektionierter Patienten, die an einer manischen Phase litten unter Praxisbedingungen im Sinne einer prospektiven, offenen, multizentrischen Studie zu prüfen.

Summary

Several randomized, controlled Trials (RCTs) prove the efficacy of Quetiapine in the treatment of manic phases in bipolar patients. However, the very methodologies of randomization and blinding can limit the generizability of RCT-results to the broader population actually seeking treatment. Many typical clinical patients would be excluded by eligibility criteria commonly used in RCTs and some eligible subjects may be unwilling to accept the limited treatment options available within a randomized study. Thus results from naturalistic studies under conditions more generalizable to clinical practise may complement those of RCTs and allow a quantitative comparison. This prospective, open, multicenter study aimed at gathering data about efficacy and safety of quetiapine in the treatment of manic phases from a large number of unselected patients under naturalistic conditions.
