Zusammenfassung
Hintergrund
In Deutschland liegen bisher keine Daten zur Leitlinienumsetzung und zum gesundheitsökonomischen Langzeitverlauf bei Patienten mit neudiagnostizierter Epilepsie vor.
Ziel der Arbeit
Die Umsetzung der Leitlinie „Erster epileptischer Anfall und Epilepsien im Erwachsenenalter“ aus 2008 soll bei Patienten mit neu diagnostizierter Epilepsie retrospektiv untersucht und der Resourcenverbrauch bei diesen Patienten dargestellt werden.
Material und Methoden
Zwischen 2008 und 2012 wurden in 6 neurologischen Facharztpraxen 128 neudiagnostizierte Epilepsiepatienten und über die Epilepsiesoftware EPI-Scout® Daten zur Therapie und zum Resourcenverbrauch erfasst.
Ergebnisse
Bei 8,6 % der Patienten bestand eine idiopathisch generalisierte Epilepsie, bei 67,2 % eine fokale Epilepsie und bei 24,2 % Patienten ein unklassifiziertes Epilepsiesyndrom. Das mittlere Alter der Patienten lag bei 57,2 ± 24,8 Jahren. Der Anteil der initialen Monotherapie mit einem zugelassenen Wirkstoff betrug 94 %, mit einem nichtenzyminduzierenden Antiepileptikum 88 % und mit einem Mittel der ersten Wahl ohne Einschränkungen in Bezug auf möglichen Schwangerschaftswunsch 58 %. Die gesamten direkten Kosten im ersten Jahr nach Diagnosestellung lagen bei 3025 € pro Patient und wurden zu 62 % durch Krankenhausaufenthalte (1875 €) und zu 29 % durch die antikonvulsive Therapie (878 €) verursacht. Die indirekten Kosten lagen bei 3400 €. Im zweiten Jahr zeigen sich signifikant niedrigere gesamte direkte Kosten (1367 €; p = 0,028), die am ehesten auf den signifikanten Rückgang der Krankenhauskosten (444 €; p = 0,009) und stabilen Kosten bei der antikonvulsiven Therapie (802 €) zurückgeführt werden können. Die indirekten Kosten lagen bei 2651 €.
Schlussfolgerungen
Die initiale Monotherapie von Epilepsie erfolgte in überwiegender Mehrzahl leitliniengerecht. Aufgrund der geringen Fallzahlen besteht bei dieser Pilotstudie jedoch nur eine eingeschränkte Aussagekraft. Es bedarf der zukünftig der Auswertung von Sekundärdaten, um allgemeine Aussagen zur Leitlinienumsetzung und Kostenentwicklung zu treffen.
Abstract
Background
In Germany, there is a lack of data on the implementation of clinical practice guidelines and health economic long-term course in patients with newly diagnosed epilepsy.
Objective
The implementation of the guideline “First epileptic seizure and epilepsy in adults” from 2008 is retrospectively investigated in patients with newly diagnosed epilepsy and resource consumption is shown in these patients.
Materials and methods
Between 2008 and 2012, a total of 128 patients with newly diagnosed epilepsy were enrolled in 6 neurological practices using the eletronic epilepsy management software EPI-Scout®.
Results
Overall, 8.6 % of patients suffered from idiopathic generalised epilepsy, 67.2 % from focal epilepsy and 24.2 % had an unclassified epilepsy syndrome. Mean age was 57.2 ± 24.8 years. Initial monotherapy was implemented in 94 % with an anticonvulsive drug licensed for monotherapy, in 88 % with a non-enzyme inducing drug and in 58 % with the drug of choice considering restrictions due to family planning. Total annual direct costs during the first year amounted to € 3025 per patient and were mainly caused by hospitalization (€ 1875, 62 % of total direct costs) and anticonvulsant treatment (€ 878, 29 % of total direct costs). The indirect cost amounted to € 3400. In the second year, there was a significant reduction of total annual costs (€ 1367; p = 0.028) due to a significant reduction in hospitalization costs (€ 444; p = 0.009). Costs for anticonvulant therapy remained stable at € 802. Indirect cost amounted to € 2651.
Conclusions
The initial monotherapy was implemented predominanly according to the clinical practice guidelines. Due to the small number of evaluated patients this pilot study has limited significance. Further studies evaluating secondary data are warranted to analyze guideline implementation and cost development on a national level.
