Zusammenfassung
Bei Patienten mit distalen intraartikulären Radiusfrakturen, Bandverletzungen oder Frakturen der Handwurzel kommt es nach verbliebener Fehlstellung oder Stufenbildung von Gelenkflächen oftmals zur Ausbildung einer schmerzhaften radiokarpalen Arthrose. An Behandlungsverfahren stehen neben der Denervation nach Wilhelm und Versteifungsoperationen auch die Proximal row carpectomy und die Korrekturosteotomie zur Verfügung. Jedes dieser Behandlungsverfahren besitzt ein definiertes Indikationsspektrum und setzt eine genaue Analyse der zugrunde liegenden Ursache und der Ausdehnung der Arthrose voraus. Die Schmerzreduktion und die Funktionalität der betroffenen Extremität sind die entscheidenden Kriterien zur Evaluierung des Behandlungserfolgs. Nicht in allen Fällen wird das vom Patienten erwartete Ziel einer guten Beweglichkeit und deutlichen Schmerzlinderung erreicht.
Abstract
In patients with distal intra-articular fractures of the radius, ligamentous lesions or fractures of the heel of the hand painful radiocarpal joint inflammation often follows residual faulty alignment or the formation of “steps” between the articulating surfaces of the joint concerned. Treatment procedures available include the Proximal row carpectomy and corrective osteotomy, in addition to denervation after Wilhelm and arthrodesis. The indications are different for each of these procedures, so that it is a detailed analysis both of the extent of the arthrosis and of its underlying causes is important. Pain relief and function of the extremity affected are the decisive criteria that should be used in evaluation of how successful the selected procedure has been. It is not always possible to fulfil the patient’s expectations of good mobility and decisive pain relief.
