Zusammenfassung
Die extraluminale Valvuloplastie kann bei geeigneten Patienten eine Alternative zu den operativen oder endovenösen Verfahren darstellen. Ziel ist eine Reparatur der terminalen und präterminalen Klappe, um die V. saphena magna erhalten zu können. Besonders geeignet sind daher Patienten mit kardiovaskulären Vorerkrankungen, einem postthrombotischen Syndrom oder einer Insuffizienz der saphenofemoralen Mündung mit Fortleitung des Refluxes in die V. saphena accessoria anterior. In den letzten 5 Jahren wurden mehrere Studien dazu publiziert. Insgesamt zeigte sich eine Erfolgsrate im Sinne einer Wiederherstellung der Klappenfunktion von über 90 % bei gleichzeitig hoher Patientenzufriedenheit. Zudem konnte eine signifikante Reduktion der Venendurchmesser nachgewiesen werden. Im Langzeitverlauf kann es jedoch zu einer erneuten inkompletten Stammveneninsuffizienz vor allem durch Perforansvenen kommen. Künftige technische Weiterentwicklungen haben ein minimal-invasives Vorgehen mit einer perkutanen Klappenreparatur zum Ziel.
Abstract
External valvuloplasty can be an alternative treatment option in selected patients instead of surgical or endovenous techniques. The function of the terminal and preterminal valves can be restored by external valvuloplasty to preserve the long saphenous vein. Especially suitable are therefore patients with pre-existing cardiovascular diseases, postthrombotic syndrome or insufficiency of the saphenofemoral junction with reflux continuing into the anterior accessory saphenous vein. Several studies have been published on this topic in the last 5 years. The data demonstrate a success rate of over 90% in the sense of a restoration of valvular function with high patient satisfaction. Moreover, a significant reduction of the diameter of the long saphenous vein has been described after treatment; however, in the long term renewed incomplete reflux can develop in the long saphenous vein mainly due to perforating veins. Future technical developments aim to repair the valves by a minimally invasive percutaneous access.
