Aortenaneurysma—Versorgung bei Vorliegen einer Hufeisenniere

Zusammenfassung

Die Versorgung eines infrarenalen Aortenaneurysmas bei konkomitantem Vorliegen einer Hufeisenniere ist ein seltenes Ereignis. Hierbei ist mit einer hohen Rate an renovaskulären Anomalien zu rechnen. Unser Patient wurde transperitoneal nach Durchtrennung des Nierenisthmus mit Interposition einer Dacron®-Tubeprothese versorgt. Zwei kräftige zusätzliche Nierenarterien, die sich erst intraoperativ exakt darstellten, wurden reimplantiert. Die verschiedenen Therapiemöglichkeiten werden anhand der gesichteten Literaturdaten dargestellt, und das eigene Vorgehen wird erörtert.

Abstract

The treatment of an infrarenal aortic aneurysm associated with a horseshoe kidney is a rare event. There is a high incidence of concomitant renovascular anomalies. Our procedure of choice in this patient was the transperitoneal approach, separation of the renal isthmus, and reconstruction by interposition of a Dacron® tube graft. Two strong additional renal arteries were reimplanted. A literature review of the different treatment options is presented and our own procedural choice is discussed.
