Zusammenfassung
Hintergrund
In der Mehrzahl der Fälle können Aderhautmelanome mittels strahlentherapeutischer Verfahren unter Erhaltung des Auges behandelt werden. Häufig kommt es jedoch nach der Behandlung zu einer Visusverschlechterung. Das Ziel der Studie war die Erstellung eines Modells zur Abschätzung der Visusprognose nach Gamma Knife Radiochirurgie, als Hilfestellung bei der Therapieentscheidung und zur Unterstützung bei der Aufklärung der PatientInnen vor der Behandlung.
Material und Methodik
Alle zwischen Juni 1992 und Mai 2010 an der Univ. Augenklinik Graz mit Gamma Knife Radiochirurgie wegen eines Aderhautmelanoms behandelten PatientInnen wurden ausgewertet. Erhoben wurden der initiale Visus und der Zeitpunkt des Visusverlustes, PatientInnen-, Tumor- und Behandlungsparameter. Ein multivariates Cox Regressionsmodell zur Bestimmung der Wahrscheinlichkeit eines Visusverlustes wurde berechnet. Um den PatientInnen schon während des Erstgesprächs bei der Therapieentscheidung eine Prognose über ihr Sehvermögen nach der Behandlung geben zu können, wurde eine Nomogramm erstellt.
Resultate
Im Untersuchungszeitraum wurden 189 PatientInnen mit dem Gamma Knife behandelt. Die Wahrscheinlichkeit eines Visuserhaltes von präoperativ besser als 0,5, 0,1 und Fingerzählen betrug fünf Jahre nach der Behandlung 13 %, 14 % und 36 %. Tumorparameter (Größe und Lage) und eine begleitende seröse Netzhautablösung waren die entscheidenden Risikofaktoren für eine Visusverschlechterung. Behandlungsparameter (Dosis, Isodose) und PatientInnenparameter (Geschlecht, Alter, systemische Erkrankungen) waren weniger wichtig. Die Güte des Nomogramms zur Vorhersage des Visusverlustes beträgt 0,7 (concordance index).
Schlussfolgerungen
Die Auswahl der für die Therapieform geeigneten PatientInnen ist der entscheidende Faktor für das Visusergebnis nach Gamma Knife Radiochirurgie. Die Behandlung sehr großer Aderhautmelanome in unmittelbarer Nähe zur Papille und mit begleitender Netzhautablösung, mit dem vorrangigen Ziel des Augenerhaltes, resultiert in schlechten Visusergebnissen. Die schwierige Therapieentscheidung zwischen dem PatientInnenwunsch nach augenerhaltender Therapie auch bei zu erwartender Sehverschlechterung, und Schmerzen sowie Kosten verursachenden Behandlungen mit schlechter Prognose kann durch eine einfachere Abschätzung des Behandlungserfolges vor der Therapie – wie es das Nomogramm ermöglicht – erleichtert werden.
Summary
Background
The majority of patients with choroidal melanoma can be treated with eye preserving methods, usually radiotherapy. However, visual acuity is frequently compromised after treatment. The goal of our study, was to create a model to estimate the likelihood of visual loss after Gamma Knife radiosurgery, to help in the therapeutic decision-making and counseling of patients before treatment.
Material and methods
We reviewed medical records and treatment plans of all patients with choroidal melanoma treated by Gamma Knife radiosurgery between June 1992 and May 2010 at Medical University Graz. Primary outcome measure was the loss of visual acuity, initially better than 20/40, 20/200 and counting fingers, over time of follow-up. Patient, tumor and treatment parameters were evaluated as possible risk factors. A multivariate Cox regression models was created, to predict conservation of visual acuity after treatment. A nomogram was designed to facilitate the calculation of the risk for visual loss.
Results
A total of 189 patients were treated with Gamma Knife radiosurgery. The probability of keeping a visual acuity of 20/40, 20/200 and counting fingers 5 years after treatment was 13, 14 and 36 %, respectively. Tumor parameters (size and location) and retinal detachment before treatment were the most important risk factors for visual loss. Treatment parameters (dose) and patient related parameters (sex, age, and systemic diseases) were less important. The nomogram facilitates the prediction of the visual outcome after treatment with an accuracy of 70 % (concordance index).
Conclusions
Proper patient selection is the most important factor for visual outcome after Gamma Knife radiosurgery. Treatment of large choroidal melanomas close to the optic disc and accompanying retinal detachments results in unfavorable visual outcome after treatment. The therapeutic decision is especially difficult if patients want to retain the eye, but severe visual loss and side effects are likely to occur after treatment. The easy to use nomogram might help to facilitate this decision by informing the patient about her/his likely prognosis.
