Zusammenfassung
Seit die perkutane Aortenklappenimplantation klinische Routine für Hochrisikopatienten geworden ist, rückt die interventionelle Therapie der Mitralklappeninsuffizienz (MI) weiter in den Fokus klinischer Forschung. Rekonstruktionsverfahren, wie Edge-to-Edge-Plastik, direkte und indirekte Anuloplastie sowie transapikale Sehnenfadenimplantation, haben bereits die Communauté-Européenne(CE)-Zulassung erhalten, werden klinisch implantiert und vergütet, lassen sich jedoch nicht auf alle Patienten mit MI anwenden. Neben weiteren innovativen kathetergestützten Rekonstruktionstherapien wurden Systeme zur „transcatheter mitral valve implantation“ (TMVI) entwickelt und sind in klinischer Prüfung. Der vorliegende Beitrag gibt einen Überblick zur präoperativen Vorbereitung, diskutiert Vor- und Nachteile von TMVI im Vergleich zu Rekonstruktionsverfahren und beschreibt die bereits bestehende Erfahrung mit einzelnen Prothesentypen. Erste Machbarkeitsstudien zeigen höhere Komplikationsraten als bei den interventionellen Rekonstruktionen, jedoch eine effektivere Behandlung der MI. Bei TMVI handelt es sich um zusätzliche Therapieoptionen für Patienten, die sich nicht für konventionelle chirurgische Verfahren eignen. Die Entscheidung zwischen TMVI und interventionellen Rekonstruktionsverfahren sollte unter Abwägung aller patientenspezifischen Kriterien im Heart-Team aus interventionell tätigen Kardiologen und Herzchirurgen getroffen werden. Für die Auswahl des richtigen Therapieverfahrens sind ein präoperatives EKG-getriggertes CT sowie eine transösophageale Echokardiographie (TEE) unumgänglich. Wird TMVI favorisiert, stehen insgesamt 6 Prothesentypen zur Auswahl, die im Rahmen von Studien implantiert werden.
Abstract
Since transcatheter aortic valve implantation has become clinical routine for high-risk patients, interventional treatment of mitral regurgitation (MR) has shifted into the spotlight of clinical research. Repair techniques such as mitral edge-to-edge repair, direct and indirect annuloplasty and transapical implantation of chordae tendineae have already gained Communauté-Européenne (CE) approval and are clinically implanted and remunerated but are not suitable for use in all patients. In addition to a broad portfolio of new transcatheter repair techniques, devices for transcatheter mitral valve implantation (TMVI) have been developed and are currently being evaluated in clinical studies. This article gives a review of the preoperative preparation, discusses the advantages and disadvantages of TMVI in comparison to reconstruction procedures and describes experiences with individual types of prosthesis. First results of feasibility studies show higher complications rates than interventional reconstruction but a more effective treatment of MR. The TMVI is a complementary treatment option for patients who are unsuitable for conventional surgical procedures. The decision between transcatheter repair or implantation techniques should currently be based on patient specific criteria assessed in an interdisciplinary heart team of cardiologists and cardiac surgeons. For selection of the correct treatment procedure, a baseline ECG-gated CT and transesophageal echocardiography (TEE) are necessary. If TMVI is favored, six different prostheses are available for use, which are implanted within clinical trials.
