Zusammenfassung
Digitale Lösungen halten Einzug in alle Bereiche der Medizin. Auch in der Arthroskopie des Kniegelenks lässt sich die Visualisierung intraartikulärer Strukturen mit Hilfe digitaler Bildverarbeitung optimieren. In diesem Artikel wird ein System (Pano™ Scope, Arthrex, München, Deutschland) vorgestellt, welches es erlaubt, aus einem 45°-Weitwinkelbild einen 30°- und einen 70°-Blickwinkel zu errechnen. Zudem ermöglicht es, den Blickwinkel per Knopfdruck intraoperativ zu ändern. Wir haben dieses neue Arthroskopie-System mit den digital einstellbaren Blickwinkeln am Sawbone-Präparat und am Kadaverpräparat getestet. Am Sawbone-Präparat oder Kadaverknie konnte gezeigt werden, dass sich insbesondere die 70°-Perspektive zur Darstellung der femoralen Insertionszone über das anterolaterale Portal und zur Darstellung der Rampenregion des Innenmeniskus eignet. Insbesondere war die 45°-Weitwinkeloptik während der durchgeführten arthroskopischen Operationen („all-inside meniscus repair“, Rekonstruktion des hinteren Kreuzbands und „suture hook repair“ einer medialen Rampenläsion) vorteilhaft, da diese einen sehr großen Überblick über das Operationsfeld erlaubte. Zusammenfassend lässt sich sagen, dass diese digitale Anwendung geeignet ist, die Visualisierung von intraartikulären Strukturen während arthroskopischer Eingriffe zu verbessern.
Abstract
Digital solutions are becoming integrated into all fields of medicine. In arthroscopy of the knee joint the visualization of intra-articular structures can also be optimized using digital image processing. This article presents a new arthroscopy system (Pano™ Scope, Arthrex, Munich, Germany) that enables a 30° and a 70° viewing angle to be calculated from a 45° wide-angle image. It also enables the viewing angle to be changed intraoperatively at the push of a button. This new arthroscope with digitally adjustable viewing angles was tested on Sawbone specimens and cadaver specimens and it could be shown that the 70° perspective is particularly suitable for viewing the femoral insertion zone via the anterolateral portal and the ramp region of the medial meniscus via a transnotch view. For arthroscopic operations, all-inside meniscus repair, posterior cruciate ligament (PCL) reconstruction and suture hook repair of a medial ramp lesion, the 45° wide-angle lens was advantageous because it allows a very large overview of the operation field. In summary, this application using digital image processing is suitable for improving the visualization of intra-articular structures during arthroscopic operations.

Video 1: Video einer arthroskopischen Versorgung einer Rampenläsion am Kadaverpräparat mit der Nahthaken-Methode über das posteromediale Portal mit verschiedenen digital prozessierten Blickwinkeln. Die Darstellung der Rampenläsion erfolgte ausschließlich über den „trans notch view“. Der Nahthaken wurde über ein posteromediales Portal eingebracht.
Video 2: Video einer arthroskopischen HKB-Rekonstruktion am Kadaverpräparat mit verschiedenen digital przessierten Blickwinkeln. Das Transplantat (Semitendinosussehne) wird mit einem „Fiber Tape“ augmentiert, welches unabhängig gespannt wird und am femoralen „Button“ fixiert ist. Zur femoralen Fixation wird ein „adjustable loop“ System (ACL Fiber Tack Tight Rope) verwendet. Die tibiale Fixation erfolgt über einen Fixationsknopf. Der Einzug des Transplantates erfolgt über das anteromediale Portal.
