Eine seltene, aber folgenschwere Rotatorenmanschettenläsion

A rare rotator cuff lesion with severe consequences

Zusammenfassung

Die isolierte muskulotendinöse Infraspinatusruptur mit begleitendem Ödem (Walch-Läsion) ist eine seltene und atypische Rotatorenmanschettenläsion. Im vorgestellten Fall eines 42-jährigen Patienten mit Nacht- und Ruheschmerzen der linken Schulter, einer ausgeprägten Atrophie der Fossa infraspinata, einem aktiven Außenrotationsdefizit von 30° und einer Kraftminderung der Außenrotation (Grad 3/5 nach Janda) zeigte sich magnetresonanztomographisch eine komplette Ruptur des M. infraspinatus am muskulotendinösen Übergang (Walch-Läsion) mit ausgedehntem Hämatom. Die Läsion wurde arthroskopisch in Seit-zu-Seit-Fadentechnik verschlossen. Sechs Monate postoperativ war der Patient schmerzfrei und mit dem Ergebnis sehr zufrieden. Die arthroskopische Rekonstruktion ermöglicht eine Wiederherstellung der Integrität des M. infraspinatus.

Abstract

The isolated musculotendinous rupture of the infraspinatus muscle with edema (Walch lesion) is a rare and atypical rotator cuff lesion. We report the case of a 42-year-old patient presenting with pain in the left shoulder especially at rest and at night, a pronounced atrophy of the infraspinatus fossa, an active external rotation deficit of 30° and an external rotation strength deficit (grade 3/5 according to the Janda scale)). Magnetic resonance imaging (MRI) showed a complete musculotendinous tear of the infraspinatus with extensive edema. The rupture was treated arthroscopically using a side-to-side suture technique. The patient was free of pain and very satisfied with the result 6 months after the operation. Arthroscopic reconstruction is able to restore the integrity of the infraspinatus muscle.
