Zusammenfassung
Hintergrund
Zur weiteren Optimierung der CO2-Laser-Stapedotomie wurden Scannersysteme entwickelt, die es ermöglichen, mit nur einer einzigen Applikation der Laserstrahlung („Ein-Schuss-Technik“) eine ausreichend große (Durchmesser von 0,5–0,6 mm), nahezu runde und gut reproduzierbare Fußplattenperforation zu erzielen.
Patienten und Methoden
255 Patienten mit Otosklerose wurden einem Primäreingriff einer CO2-Laser-Stapedotomie mit dem SurgiTouch™-Scanner unterzogen. Die chirurgische Technik, die Effektivität der Laseranwendung und das erzielte Hörvermögen werden dargestellt.
Ergebnisse
Mit einer einzigen Laserapplikation konnte bei 68% der Eingriffe eine ausreichend große Perforation erzielt werden. Bei 14% der Patienten war eine 2. Laserapplikation auf dieselbe Stelle mit dem Scannersystem notwendig. Zusätzliche leicht überlappende Laserapplikationen ohne Scannersystem am Perforationsrand zur Vergrößerung einer zu kleinen Perforation waren bei weiteren 18% der Patienten erforderlich. Die postoperativen Ergebnisse zeigen deutlich, dass bei dieser Technik keine Gefahr von laserinduzierten Innenohraffektionen besteht.
Schlussfolgerung
Der CO2-Laser in Kombination mit modernen Scannersystemen ist sehr gut für die Anwendung in der Steigbügelchirurgie geeignet.
Abstract
Background
In order to further optimize the surgical technique with CO2 laser in stapes surgery, a scanner system was used to obtain a footplate perforation of 0.5–0.6 mm with only one laser application (“one-shot” stapedotomy).
Patients and methods
A total of 255 patients with otosclerosis were submitted to primary CO2 laser stapedotomy with a SurgiTouch™ scanner. This study discusses the surgical technique and clinical results.
Results
An adequately large perforation diameter could be achieved with a single shot in 68% of the patients treated. In 14% of the patients, a second laser application at the same site was necessary. In 18% the perforation had to be enlarged by several slightly overlapping laser applications without using the scanner. There was no evidence of laser dependent inner ear affections.
Conclusion
CO2 laser, combined with modern scanner systems, is well suited for application in stapes surgery.
