Zusammenfassung
Hintergrund. Maligne Onkozytome der großen Speicheldrüsen sind seltene epitheliale Tumoren, sie entsprechen weniger als 1% aller Speicheldrüsenkarzinome. Sie sind meist in der Glandula parotis, nur zu ca. 10% in der Glandula submandibularis lokalisiert und treten etwa gleich häufig bei Frauen und Männern auf. Der Altersgipfel liegt im 5.–6. Lebensjahrzehnt. Onkozyten sind stark vergrößerte Epithelzellen mit kräftiger granulärer Eosinophilie des Zytoplasmas, bedingt durch dicht gepackte und cristareiche Mitrochondrien. Die maligne Transformation aus benignen Onkozytomen ist am Nebeneinander von Onkozytenresten und atypischen onkozytären Karzinomanteilen zu erkennen. Die Tumoren besitzen keine Kapsel und zeigen ein lokal invasives Wachstum.
Patient. Bei einem 59-Jährigen zeigte sich ein onkozytäres Adenokarzinom am Vorderrand der linken Glandula submandibularis mit angrenzendem Lymphknoten.
Ergebnisse und Schlussfolgerung. Klinische und histologische Besonderheiten des Onkozytoms werden herausgestellt. Ein Literaturvergleich gibt einen Überblick über bisher erschienene Publikationen. Die Therapie der Wahl besteht im operativen Vorgehen sowie ggf. postoperativer Strahlenbehandlung, gefolgt von engmaschigen klinischen Kontrollen.
Abstract
Background. Malignant oncocytic tumors rarely occur in major salivary glands and generally account for <1% of all salivary tumors. There is no difference of both sexes and appearing mainly between 50 and 60 years of age. They comprise oncocytes, which are epithelial cells with abnormally formed cytoplasm and acidophilic granular cytoplasma. In the process of the malignant transformation the coexistence from oncocytic rests and untypical oncocytic carcinoma portions can be seen. The tumors show no capsule and an infiltrative local growth with perineural and vascular expansions.
Patient and method. A rare case of a 59-year-old man with malignant oncocytoma is presented. The tumor was found in the left submandibular gland with one enlarged lymph node near by.
Result and conclusions. A general overview of the available literature and the histological classification of oncocytomas is given. Therapy consists of surgical resection and radiotherapy. Close clinical controls are necessary.
