Zusammenfassung
Das Inkrafttreten des Infektionsschutzgesetzes zum 1.1.2001 hat die Bedeutung der Infektiologie als interdisziplinäres Fachgebiet hervorgehoben. Sie ist zugleich Grundlagenwissenschaft für die Dermatomikrobiologie. Für die Hautmykosen beispielsweise zeigt das Achilles-Projekt, dass diese noch häufiger als bisher angenommen auftreten. Entscheidenden Einfluss auf die Diagnostik haben inzwischen molekularbiologische Verfahren. Hierdurch können neue Mikroorganismen (z. B. Helicobacter pylori) entdeckt und bekannte Erreger (z. B. Chlamydien, Lyme-Borreliose) noch eindeutiger klassifiziert werden, was jedoch noch keinen Beweis für den Kausalzusammenhang liefert. Das molekulare Verständnis von Pathomechanismen hat gleichwohl zur Entwicklung neuer Therapien (z. B. Behandlung von Kandidosen, Herpesinfektionen) geführt. In einem Überblick wird der aktuelle Stand von Diagnostik und Therapie in der Dermatomikrobiologie, jeweils unterschieden nach Mykologie, Bakteriologie und Virologie, skizziert und erläutert. Dabei erscheint es sinnvoll, das hier als Dermatomikrobiologie vorgestellte Fachgebiet im Lichte einer dermatologischen Infektiologie zu betrachten.
Abstract
The new German laws dealing with protection against infectious diseases (Infektionsschutzgesetz of 1.1.2001) have reconfirmed the importance of microbiology as a basic science and infectiology as a multi-disciplinary clinical discipline. Concerning dermatological diagnosis and therapy, the implementation of molecular biological techniques has a decisive influence. The discovery of new (e.g. Helicobacter pylori) and the better detection of known organisms (e.g. Chlamydia, Borrelia) suggested findings in questions of causality and therapeutic developments. Concerning the infections of the skin this synopsis provides the state of the art of diagnostic process and treatment in mycology, bacteriology and virology.
