Zusammenfassung
Die subtotale Schilddrüsenresektion beim benignen Struma ist mit einem hohen Rezidivrisiko behaftet. Zudem führt die relativ hohe Rate postoperativer Karzinomzufallsbefunde zu einer Vielzahl von Komplettierungsthyreoidektomien. International ist es daher zu einem Paradigmenwechsel in der Schilddrüsenresektionsstrategie von der subtotalen Resektion zur ausgedehnten oder totalen Thyreoidektomie gekommen, mit der Folge, dass sich das Komplikationsrisiko von der Schilddrüse auf die Stimmbandnerven und Nebenschilddrüsen verlagert hat. Eine einmal eingetretene Komplikation an diesen Strukturen ist für den Betroffenen in der Regel wesentlich gravierender als an der Schilddrüse selbst. Störungen der Rekurrens- und Nebenschilddrüsenfunktion bei ausgedehnter Resektion bestmöglich zu vermeiden, ist daher heute das Hauptziel der Operation. Bei entsprechendem Training unter ggf. technikunterstützter Berücksichtigung der Anatomie und Funktion der Stimmbandnerven (intraoperatives Neuromonitoring) und Nebenschilddrüsen (intraoperative Parathormonbestimmung) liegt das Komplikationsrisiko bei ausgedehnten Resektionen mit Rekurrens- und Nebenschilddrüsenpräparation jedoch nicht höher als bei der klassischen subtotalen Resektion ohne Rekurrens- und Nebenschilddrüsendarstellung.
Abstract
Subtotal thyroid resection in the case of benign goiter carries a high risk of recurrence. The relatively high rate of postoperative cancer findings also leads frequently to secondary completion thyroidectomy. Internationally therefore the paradigm has shifted from subtotal resection to extended or total thyroidectomy, which shifts the risk to the vocal cords and parathyroids, where as a rule complications are much worse than those to the thyroid. The main surgical goal therefore is avoiding as far as possible recurrent laryngeal nerve palsy and disturbances to parathyroid function following extended resection. With adequate training and technical intraoperative aids for preserving anatomy and function of the vocal cords (neuromonitoring) and parathyroids (parathyroid hormone monitoring) the risk of complications after extended resection is not significantly higher than after classic subtotal resection without identification of the nerve and parathyroids.
