Exakte Reposition und sichere Retention des tibialen knöchernen Ausrisses des hinteren Kreuzbandes über dorsalen Zugang zum Kniegelenk.
Indikationen
Knöcherner tibialer Ausriss des hinteren Kreuzbandes.
Kontraindikationen
Ligamentärer Ausriss des hinteren Kreuzbandes. Schlechter Weichteilmantel am Kniegelenk beugeseitig.
Operationstechnik
Dorsaler Zugang über Musculus gastrocnemius medialis, welcher nach Inzision der Unterschenkelfaszie nach lateral stumpf mobilisiert und mit dem Venenhaken gehalten wird. Weghalten des Musculus semimembranosus nach medial mit dem Langenbeck-Haken. Darstellung des knöchernen Ausrisses des hinteren Kreuzbandes nach subperiostaler Mobilisation des Musculus popliteus nach lateral und Inzision der dorsalen Kniegelenkkapsel. Reposition des knöchern ausgerissenen tibialen Ansatzes des hinteren Kreuzbandes. Stabilisierung des Fragments mit vorgeformter Krallenplatte (Drei- bis Vier-Loch-Kleinfragment- Drittelrohrplatte).
Weiterbehandlung
6 Wochen Teilbelastung mit 15 kg Körpergewicht an Unterarmgehstützen, maximale Beugung für 6 Wochen bis 90°. Eine Kniegelenkorthese ist nicht notwendig. Die Verwendung eines EMT-Geräts ist zur Vermeidung einer Muskelatrophie empfehlenswert.
Ergebnisse
Sechs Patienten (vier Frauen, zwei Männer) mit einem knöchernen Ausriss des hinteren Kreuzbandes wurden über den posteromedialen Zugang mit der Krallenplatte versorgt.
Abstract
Objective
Posteromedial approach to the knee. Exact reduction and fixation of avulsed tibial attachment of posterior cruciate ligament with small-fragment plate.
Indications
Avulsed tibial attachment of posterior cruciate ligament.
Contraindications
Ligamentous rupture of the posterior cruciate ligament. Local soft-tissue problems.
Surgical Technique
Posteromedial approach with mobilization and retraction of the medial gastrocnemius muscle. Subperiosteal detachment of the popliteal muscle. Identification of fracture site. Longitudinal incision of the posterior capsule of the knee. Debridement of the fracture site, reposition and stabilization with small-fragment plate.
Postoperative Management
Partial weight bearing with 15 kg for 6 weeks, limitation of knee flexion to 90° for 6 weeks. A special joint orthesis is not necessary. The use of an EMT unit is recommended to prevent muscular atrophy.
Results
Six patients (four female, two male) with avulsed tibial attachment of posterior cruciate ligament were treated using the posteromedial approach.
