Zusammenfassung
Seit den 1980er Jahren hat sich in der Behandlung des Analkarzinoms ein grundlegender Wandel vollzogen. Bis dahin war die abdominoperineale Rektumexstirpation mit definitivem Kolostoma die Standardtherapie. Beginnend mit den Studien von Nigro hat sich die simultane Radiochemotherapie mit Erhaltung des Kontinenzorgans als Primärtherapie durchgesetzt. Die Chirurgie spielt als Therapie der ersten Wahl nur in einem kleinen Tumorkontingent eine Rolle: als lokale Tumorexzision bei gut differenzierten Analkanalkarzinomen mit einer Tumorgröße bis 1 cm (T1 N0 G 1–2) und bei gut differenzierten Analrandkarzinomen T1–2 N0 G1–2. Alternativ ist in diesen Fällen auch eine primäre Radiochemotherapie möglich. Bei der Radiochemotherapie wurde in zahlreichen Studien versucht, das optimale Vorgehen herauszufinden. Bei der Standardradiochemotherapie wird der Tumor einschließlich der Leisten- und Beckenlymphknoten mit einer Gesamtdosis von 50,4–59,4 Gy bestrahlt. Simultan erfolgt eine Chemotherapie mit 5‑Fluorouracil in den Tagen 1–4 und 29–32 als Dauerinfusion und mit Mitomycin C an den Tagen 1 und 29 als i.v.-Bolus. Mit keiner anderen Kombinationstherapie konnte für die Patienten ein größerer Gewinn erreicht werden. Die Hauptrolle der Chirurgie besteht in der Behandlung des Rezidivtumors nach Radiochemotherapie und des Resttumors nach dieser Behandlung. Die chirurgische Therapie erfolgt mittels der abdominoperinealen Rektumexstirpation und der Anlage eines definitiven Kolostomas (Salvagetherapie), ggf. kombiniert mit plastischen Eingriffen. Des Weiteren gibt es Indikationen für die alleinige Anlage eines temporären oder permanenten Anus praeter sigmoidalis. Das nichtmetastasierte Analkarzinom ist ein gut behandelbarer Tumor mit guter Prognose.
Abstract
Since the 1980s the treatment of anal cancer has fundamentally changed. Whereas up until then abdominoperineal rectal extirpation with definitive colostomy was the established therapy, it was replaced by simultaneous continence-preserving chemoradiotherapy as primary treatment, beginning with the studies by Nigro. Indications for primary surgical treatment are nowadays limited to local excision of the tumor in well-differentiated tumors up to 1 cm in size (T1N0G1-2) and well-differentiated T1-2N0G1-2 tumors of the anal rim. In both cases primary chemoradiotherapy is also feasible. Several studies were conducted to find the optimal regimen for chemoradiotherapy. It consists of radiation of the primary tumor and inguinal as well as pelvic lymph nodes with a total dosage of 50.4–59.4 Gy and simultaneous chemotherapy with 5‑fluorouracil (days 1–4 and 29–32) and mitomycin C (days 1 and 29) as i.v. bolus. No other combination treatment could achieve a better outcome for the patients. An abdominoperineal rectal extirpation with a definitive colostomy (salvage therapy) as surgical treatment mainly figures in the treatment of residual or recurrent disease after primary chemoradiotherapy, if necessary, combined with plastic surgery. In some cases, a temporary or permanent sigmoid end colostomy alone is indicated. Non-metastatic anal cancer responds well to treatment and shows a good prognosis.
