Zusammenfassung
Zur Ermittlung einer geeigneten Untersuchungstechnik wurden vergleichende Untersuchungen desQuick-Wertes und der partiellen Thromboplastinzeit im Plasma und Kapillarblut an 30 Säuglingen, 75 Kindern und Erwachsenen ohne bekanntes Blutungsübel sowie 10 Patienten mit Hämophilie und 10 Patienten unter oraler Antikoagulantienbehandlung durchgeführt.
Entsprechend dem Ziel und der weitgehenden Fragestellung der Untersuchung, die Brauchbarkeit und Anwendbarkeit von Kapillarblutproben zu testen, ergaben sich in Vergleichsserien gut reproduzierbare Ergebnisse. Dabei erwies sich als besonders geeignete Punktionsstelle bei Säuglingen, auch bei wiederholter Blutentnahme, die Ferse. Andererseits fanden sich bei mehrfachen Blutentnahmen aus der Fingerbeere ungenaue Ergebnisse.
Für dieQuick-Wertbestimmung aus Kapillarblut wurde wegen starker Hämatokritschwankungen in der Einzelprobe die Aufstellung von Eichkurven mit konstantem Hämatokritwert erforderlich. Diese Kurven wurden vorgestellt.
Für die partielle Thromboplastinzeit ergaben sich bei der Bestimmung in Kapillarblut signifikant längere Werte als im Plasma bei guter Korrelation. Diese Bestimmung ist nicht von dem Hämatokritwert abhängig.
Summary
Prothrombin time and partial Thromboplastin time were compared to each other in plasma and capillary blood. 30 infants in first year of life and 75 older persons without any symptoms of hemorrhagic disease were investigated. 10 patients with hemophilia A or anticoagulant therapy were included in the study.
Capillary blood methods showed a good reliability. Taking blood of the heel gave best results, while the tip of the finger seems to be unreliable, especially if several samples are gained.
In determination of prothrombin time several calibration curves for varying hematocrits are used, which are shown in the paper.
Partial thromboplastin time is significantly prolonged in our capillary blood methods nevertheless the obtained values show a good reliability and constant results.
