Zusammenfassung
Die Thrombophlebitiden und davon besonders diejenigen, die erstmalig vonQuincke als gutartige intrakranielle Drucksteigerung beschrieben worden sind, bedeuten besonders im Kindesalter und bei Frauen ein oft schwieriges differentialdiagnostisches Problem. Zunächst werden diejenigen Thrombophlebitiden dargestellt, die zu fortschreitender erheblicher Drucksteigerung führen, das sind vor allem die, welche den Sinus longitudinalis superior betreffen. Anschließend wird das von den Angelsachsen als otogene Hydrocephalie bezeichnete Krankheitsbild beschrieben und das Syndrom der corticalen Thrombophlebitis, die meist postpuerperal auftritt, dargestellt. Schließlich wird an das von einigen Autoren beschriebene einseitige Hirnödem infolge Thrombose der Carotis interna oder der A. cerebri media erinnert. Diese ohnehin sehr gefährdeten Kranken sollten keinesfalls operativen Belastungen ausgesetzt werden.
Im Rahmen der pseudotumoralen Encephalitiden beschreibt der Autor die Hirndrucksymptome, die bei Encephalitiden verschiedener Ätiologie beobachtet werden, manchmal auch bei subakut sich entwickelnden Demyelinisierungskrankheiten. Tumoren im Bereich der Brücke und der Hirnschenkel können ein ähnliches Bild wie die perivenöse Encephalitis verursachen. Die akute, günstig verlaufende Ataxie des Kindesalters muß differentialdiagnostisch manchmal von einem Tumor der hinteren Schädelgrube abgegrenzt werden. Das gleiche Bild kann von bestimmten Polyradikuloneuritiden vom TypGuillain-Barré und selbst von der Poliomyelitis mit albumino-zytologischer Dissoziation hervorgerufen werden.
Die subakute sklerosierende Leukencephalitis kann einen Tumor des Scheitel- und Angularisgebietes und die akute nekrosierende Encephalitis einen Temporallappenabszeß vortäuschen. Manche mit Drucksteigerung einhergchenden Encephalopathien sind endokriner Genese, am häufigsten nebennierenbedingt; sie sollten besser bekannt sein. Andere stehen im Zusammenhang mit den weiblichen Geschlechtsorganen und der Schwangerschaft.
Schließlich wird daran erinnert, daß die Megalencephalie, die man bei der spongiösen Entartung des Gehirns sieht, manchmal einem suprasellären Tumor ähneln kann.
Summary
Cerebral Thrombophlebitis and in particular such cases as were described first byQuincke under the description of benign intracranial hypertension quite often create a difficult diagnostic problem especially in children and in the female. The author classifies the thrombophlebitides according to the degree in which they produce a syndrome of progressive or very severe intracranial hypertension; chief among these are cases with involvement of the superior longitudinal sinus. He next recalls the clinical picture described by the term “otitic hydrocephalus”; the syndrome of cortical thrombophlebitis which occurs most commonly in the puerparium, and, on the basis of personal observation, shows how the occurrence of a superior longitudinal sinus thrombosis might not be recognised. Finally he draws attention to the cases recorded by some writers of unilateral cerebral oedema due to thrombosis of the internal carotid or middle cerebral arteries. He points out that these cases are usually too ill for operation.
Under the name of “Pseudotumour Encephalitis” the author describes the results of disturbed intracranial pressure observed in Encephalitides of different aetiology, sometimes also in subacute demyelinating diseases and in tumours of the pons and brain stem which could be mimicked by a perivenous type of Encephalitis. The acute Ataxia of children with a favourable outcome can sometimes create a problem in differential diagnosis with a tumour of the posterior fossa. The same picture can arise from undoubted radicular polyneuritides of theGuillain-Barré type and occasionally from Poliomyelitis with albumino-cytological dissociation.
Subacute sclerosing leukoencephalitis can pass as a tumour of the parieto-angular region and the acute necrotising encephalitis can imitate a temporal lobe abscess. Some encephalopathies with elevated intracranial pressure are of endocrine origin, chiefly from adrenal pathology; these cases ought to be better known. Other endocrine encephalopathies are associated with disorders of the female reproductive organs and with pregnancy. The author finally points out that the enlarged head which occurs with spongy degeneration of the brain may simulate a suprasellar tumour.

Le tromboflebiti, e in modo speciale quelle descritte per la prima volta daQuincke come aumento benigno della pressione endocrania, presentano, sopratutto nei bambini e nelle donne, un difficile problema di diagnosi diffenziale. Vengono anzitutto descritte quelle tromboflebiti, che comportano un forte e progressivo aumento della pressione, e che corrispondono sopratutto a quelle che colpiscono il seno longitudinale superiore. Successivamente è descritto il quadro clinico detto dagli anglosassoni idrocefalo otogeno, e la sindrome della tromboflebite corticale, di solito di origine puerperale. Per ultimo viene ricordato l'edema unilaterale descritto da alcuni autori e dipendente da trombosi della carotide interna o della cerebrale media. Questi ultimi malati sono da considerare come molto gravi e da non sottomettere ad interventi chirurgici.
Nel quadro delle encefaliti pseudotumorali l'autore descrive gli aumenti della pressione, che si osservano nelle encefaliti di varia origine, e talora anche nelle malattie demielinizzanti a decorso subacuto.
Tumori del ponte e del peduncolo cerebrale possono simulare la sindrome da encefalite perivenosa. L'atassia acuta benigna dell'infanzia deve essere talora differenziata clinicamente da un tumore della fossa crania posteriore. Un quadro analogo puó essere mentito da alcune poliradiculomieliti del tipoGuillain-Barré, e persino da poliomieliti con dissociazione albumino-citologica.
La leucoencefalite subacuta sclerosante può simulare un tumore parieto angolare e quella acuta necrosante un ascesso temporale.
Alcune encefalopatie sono di origine endocrina, spesso surrenale, e dovrebbero essere meglio conosciute. Altre sono in rapporta con gli organi sessuali femminili e la gravidanza.
Viene per ultimo ricordato che la megaencefalia, che si osserva nella degenerazione spugnosa del cervello, può talora dare sintomi analoghi a quelli di un tumore sellare.
Resumen
Las tromboflebitis y en particular los cuadros descritos bajo el nombre de hipertensión intracraneal benigna, cuya descripción original se debe aQuincke, presentan, desde el punto de vista del diagnóstico diferencial con los tumores, sobre todo en la infancia y en la mujer, un problema a menudo dificil. El autor expone primero las trombosis venosas intracraneales que semejan a los síndromes de hipertensión intracraneal progresiva o brusca, cuando aquellas interesan al seno longitudinal superior especialmente. A continuación menciona el cuadro descrito por los autores ingleses bajo el nombre de hidrocefalia otítica, el síndrome de las tromboflebitis corticales, sobre todo de aparición post-partum y a propósito de una observación personal dice cómo el diagnóstico de trombosis del sono longitudinal superior puede pasar inadvertido. Menciona después las descripciones hechas por algunos autores del edema cerebral unilateral con trombosis de la carótida interna ó de la arteria cerebral media y la conveniencia de no someter a éstos enfermos, ya lábiles, a una intervención quirúrgica.
En el cuadro de la encefalitis pseudotumoral describe las manifestaciones de hipertensión observadas en éstas, cualquiera que sea su etiología; a veces en las enfermedades desmielinizantes de evolución subaguda. Los tumores de la protuberancia y del territorio del pedúnculo pueden ocasionar cuadros similares a encefalitis de tipo perivenoso. La ataxia aguda del niño, de evolución favorable, puede en algunos casos, plantear el diagnóstico diferencial con un tumor de la fosa cerebral posterior. El mismo cuadro puede ser causado por ciertas poliradiculoneuritis del tipo deGuillain- Barré y también por las poliomielitis con disociación albúmino-citológica.
La leucoencefalitis esclerosante subaguda puede impresionar como un tumor de la región supramarginal y angular y la encefalitis necrosante aguda como un absceso del lóbulo temporal. Ciertas encefalopatías hipertensivas son tal vez de origen endocrino, particularmente de origen suprarrenal y todas ellas deben ser mejor conocidas. Otras están relacionadas con los organos genitales de la mujer y el embarazo. Finalmente dice que la megalencefalia, que se observa en la degeneración esponjosa del cerebro puede, en ciertos casos, simular un tumor supraselar.
