Summary
An investigation was made to study the changes of calcium, phosphorus, potassium, sodium and chloride during exchange transfusions, using citrated or heparinized blood. Two comparable groups of newborns with neonatal hyperbilirubinemia were examined. Though, taking much calcium (2 ml calcium gluconat 10% per 50 ml donor blood) to neutralize the effect of citric-acid and sodium-citrat in the donor blood, calcium didn't affect other electrolytes. Hypocalcemia and hypopotassemia which were found after exchange transfusion using heparinized blood were shown to be pseudo-changes. After exchange transfusion with citrated blood a small decrease of serum-chloride levels was found as a sign of balancing the acidity which occurs during exchange transfusion. Otherwise the decrease of chloride could be an effect of the transfused donor blood, because the level of plasma-chloride of stored citrated blood is low. A statistical confidence was found between the means of the electrolyts before and after exchange transfusion in the same group and in view of the control-group. Citrated blood (not older than 3 days) transfused with a high quantity of calcium-gluconat — applied often and in rather little portions —has the same effect as it is well known from heparinized donor blood. No serious or slight incidents were observed during and after exchange transfusion since we took calcium-gluconat in a sufficient relation to sodium citrat and citric acid.
Zusammenfassung
Die Veränderungen einiger Elektrolyte (Calcium, Phosphor, Kalium, Natrium, Chlor) bei Blutaustauschtransfusionen wegen einer neonatalen Hyperbilirubinämie wurden untersucht, die durch Heparin- oder Citratblut als Spenderblut entstehen. Die Kontrolle erfolgte an zwei vergleichbaren Gruppen neugeborener Kinder. Obwohl bei der Austauschtransfusion mit Citratblut hohe Calciumgaben verwendet worden waren (2 ml 10% Calciumgluconatlösung auf 50 ml Spenderblut), konnte ein Einfluß auf andere Serumelektrolyte nicht gefunden werden. Die wider Erwarten beobachteten Hypocalcämien und Hypokaliämien nach Austauschtrans-fusionen mit Heparinblut erwiesen sich als Pseudoveränderungen, die den Wert der Heparinkonserve nicht beeinträchtigen. Bei Verwenden von Citratblut trat als wichtigste Elektrolytveränderung beim Kind eine geringe Hypochlorämie auf. Alle vorhandenen Unterschiede der Mittelwerte der Elektrolytkonzentrationen vor und nach der Austauschtransfusion innerhalb des gleichen Kollektivs und gegenüber der Kontrollgruppe ließen sich statistisch signifikant sichern. Bei Verwendung von frischem Citratblut (nicht älter als 3 Tage) und von hohen, citratgerechten Calciumdosen, die in kleinen, häufigen Portionen verabreicht werden, ist Citratblut dem Heparinblut als Spenderblut für die Austauschtransfusion nicht unterlegen. Zwischenfälle, auch leichterer Art, während oder nach einer Austauschtransfusion wurden seit dem Verwenden citratgerechter Calciummengen nur noch sehr selten beobachtet.
