Zusammenfassung
Eine Stomaanlage stellt immer einen erheblichen Einschnitt für den Patienten dar und hat in der Regel gravierende physische, mentale und psychische Implikationen. Umso wichtiger ist eine möglichst umfangreiche Aufklärung im Vorfeld, wobei das Ausmaß der Aufklärung patientenindividuell gestaltet werden muss. Insbesondere beim permanenten Stoma sind die Anforderungen an die Aufklärung noch höher. Grundsätzlich ist die Einbindung von Angehörigen sehr wünschenswert, da diese häufig auch die Nachsorge bzw. Stomaversorgung mit übernehmen müssen. Außer im unvorhergesehenen Notfall muss präoperativ eine individuelle Stomalokalisation durchgeführt und angezeichnet werden. Dabei sind anatomische Landmarken, aber auch Patientenpräferenzen zu beachten. Ideal ist eine Zusammenarbeit mit spezialisierten Stomapflegekräften sowohl präoperativ als auch in der Nachsorge. Eine dauerhafte Nachsorge sowohl pflegerisch als auch medizinisch muss für jeden Stomaträger gewährleistet werden, da in der überwiegenden Mehrheit der Fälle kleinere oder auch größere Probleme auftreten können. Die moderne Stomaversorgung bietet nahezu für jeden Patienten bzw. für jedes Problem eine Lösung. Sehr gute Unterstützung leisten Patientenorganisationen (z. B. ILCO), sodass jedem Stomapatienten und seinen Angehörigen die entsprechende Information bereitgestellt werden kann.
Abstract
An ostomy always represents a significant change in life for the patient and usually has severe physical, mental and psychological implications. Therefore, it is of utmost importance to preoperatively provide as much information as possible, although the extent of the explanations must be patient specific. Especially with a permanent stoma, the requirements for patient education are even higher. Furthermore, the involvement of relatives is principally very desirable as they are often involved in the aftercare and care of the stoma. Except in the case of an unexpected emergency, an individual stoma localization and marking must be performed preoperatively. In this respect anatomical landmarks as well as patient preferences have to be taken into account. Ideally, enterotherapists should attend to the patient both preoperatively and during follow-up. A permanent nursing care and medical follow-up must be ensured for every person with a stoma, since in the vast majority smaller or larger problems can occur. Modern ostomy treatment provides a solution for almost every patient and every problem. Patient organizations (e.g. the Ileostomy-Colostomy Association, ILCO) provide very good support so that every stoma patient and their relatives should be provided with the appropriate information.
