Zusammenfassung
Hintergrund
Immunglobulinen wird ein lindernder Effekt auf die Ausprägung der strahlenbedingten Dermatitis und Mukositis nachgesagt. Alle bisherigen Untersuchungen hierzu basieren auf empirischen Mitteilungen bzw. Fallberichten, zumeist anhand subjektiver Kriterien. In der vorliegenden Arbeit berichten wir über telethermographisch gemessene Wärmeabstrahlungen der sich im Laufe der Bestrahlungsserie entwickelnden Dermatitis bei Patientinnen, die wegen eines Mammakarzinoms in brusterhaltender Technik operiert und adjuvant bestrahlt wurden. Durch Strahlung freigesetzte Mediatoren bewirken unter anderem eine Vasodilatation von Hautgefäßen. Dadurch wird der Blutzufluß erhöht, und es kommt zu einer Temperaturerhöhung und Rötung dieses Hautareals. Wenn Immunglobuline auf die vasodilatatorische Komponente der Entzündung einen Einfluß haben, müßte sich das Wärmeabstrahlungsverhalten der mit Immunglobulin behandelten Patienten von dem einer Kontrollgruppe unterscheiden.
Patienten und Methode
16 Patientinnen wurde vor der ersten Bestrahlung 10 ml und nach der fünften und zehnten Bestrahlung (entsprich 0 Gy, 10 Gy und 20 Gy) jeweils 5 ml Beriglobin® intragluteal injiziert. Sowohl vor Bestrahlungsbeginn als auch nach jeweils 10 Gy applizierter Dosis (bis zur Zieldosis von 50 Gy) wurde der Oberkörper der Patientinnen thermographisch untersucht. Hierbei wurden die Temperaturverläufe der bestrahlten Oberkörperhälften sowohl mit den nichtbestrahlten Gegenseiten als auch mit den bestrahlten und nichtbestrahlten Oberkörperhälften einer Kontrollgruppe, bestehend aus 20 Patientinnen, die kein Immunglobulin gesprizt bekamen, verglichen.
Ergebnisse
Die Patientinnen, die Immunglobuline injiziert bekamen, wiesen, im Vergleich mit der Kontrollgruppe, kein unterschiedliches Wärmeabstrahlungsverhalten auf (Tabelle 1, Abbildung 1). Sowohl Ausgangstemperatur der Hautwärme als auch Temperaturverlauf während der Behandlung und die Endtemperatur bei Behandlungsschluß waren in beiden Gruppen identisch. Auch der relative Temperaturanstieg (Temperatur der bestrahlten Seite minus Temperatur der Gegenseite) war in beiden Behandlungsgruppen gleich (Tabelle 2, Abbildung 2).
Schlußfolgerung
Immunglobuline (Beriglobin®) haben auf das Wärmeabstrahlungsverhalten der sich während einer Bestrahlungsserie entwickelnden Dermatitis bzw. Strahlenerythems keinen Einfluß. Daraus resultiert der Rückschluß, daß sie auch keine Wirkung auf den vasodilatatorischen Anteil dieser Entzündung und damit auf die vasodilatatorisch hervorgerufe Hautrötung ausüben.
Abstract
Background
Immunoglobulines are supposed to have a soothing effect on the degree of dermatitides and mucositides caused by irradiation. Research so far has been based on empiric information or reports of case studies mostly basing on subjective criteria. This study reports on objective thermographic measuring of heat radiation of dermatitides developing in the course of radiotherapy in case of female patients receiving postoperative radiotherapy after breast preserving operation on mamma carcinoma. Idea behind this is that immunglobulines affect inflammations of skin induced by irradiation, then also one of theses parameters, in this case heat, would be subject to change.
Patients and Method
Sixteen patients received 10 ml Beriglobin® before first and 5 ml after 5th and 10th radiation (corresponds to 0 Gy, 10 Gy and 20 Gy) as intragluteal injection. Before starting with radiation as well as after 10 Gy respectively applied, upper bodies of patients were examined thermographically until final dose of 50 Gy was reached. Here temperature progress on upper body halves exposed to irradiation was compared with the untreated halves as well as with upper bodies of control group consisting of 20 patients who had received no immunoglobuline injections.
Results
Patients who received immunoglobuline injections showed no differences in their skin temperature progress in comparison with the control group (Table 1, Figure 1). Skin temperature at the beginning as well as temperature progress during treatment and temperature at the end of treatment were identical in both groups. Also the relative temperature increase (temperature of body half subject to irradiation minus temperature of the other body half) was identical in both therapy groups (Table 2, Figure 2).
Conclusion
Immunoglobulines (Beriglobin®) do not influence the course of thermal radiation due to dermatitis developing during a series of irradiation treatments. This results in the conclusion, that immunoglobulines do not have any influence in the vasodilatative part of dermatitis and vasodilatative redness respectively.
