Zusammenfassung
Bei 187 konsekutiven Fällen von Kolonkarzinom kranial des Promontoriums betrug die Resektionsrate 91,4%, die Letalität der Kolonresektion 2,3%, die Dehiszenzrate 2,9%, obwohl 57,9% der Patienten über 70 Jahre alt waren und bereits 60,3% im Stadium Dukes C oder D. Die Anastomosierung erfolgte ausschließlich durch einreihige manuelle Naht mit vollsynthetischem resorbierbarem Nahtmaterial. Die nach Kaplan-Meier berechneten 5-Jahres-Heilungen betrugen für die Stadien Dukes A bis D 100, 78, 40 und 0%. Geschlechtsspezifische Unterschiede der 5-Jahres-Ergebnisse sind zum Teil auf die größere natürliche Lebenserwartung der Frauen zurückzuführen. Die Ergebnisse der Therapie des Kolonkarzinoms haben sich in den letzten Jahren ausschließlich durch die optimale Anwendung allgemeinchirurgischer und anästhesiologischer Prinzipien verbessert, die auch für inkurable Fälle eine sichere und wenig belastende palliative Wirkung garantieren.
Summary
In 187 consecutive cases of carcinoma of the colon proximal of the promontory resection rate was 91.4%, operative mortality 2.3%, dehiscence rate 2.9%, although 57.9% of patients were ≥70 years and 60.3% were Dukes stages C or D. Anastomoses were performed exclusively by a single layer hand-sutured technique using synthetic absorbable suture material. Kaplan-Meier life table survival was 100, 78, 40 and 0% for Dukes stages A to D respectively. Sex-linked differences in 5-year-survival are due to a greater life-expectancy in women. In the recent past, progress in the therapy of colonic cancer has been achieved exclusively by optimal application of general surgical and anesthesiologic principles, which also provide reliable and safe palliation for incurable cases.
