Summary
Electron microscopic investigations reveal thickened basement membranes of the capillaries of the lower leg-muscles in most older patients; only two old patients had thin basement membranes. The lesions are generally smaller in the examined muscles of the abdominal wall and forearm. Thick basement membranes are also found in a third of the patients of younger ages with chronic diabetes. In the basement membrane inclusions are seen; lipopigment and increased numbers of cytosomes are observed in the endothelial cells. Much lipopigment and rarely large myelin-structures or inclusions with globular particles or filament-like structures are found in the cells of skeletal muscles.
Zusammenfassung
Die elektronenmikroskopische Untersuchung von Skeletmuskelcapillaren ergibt eine Grundhäutchenverdickung an den Unterschenkelcapillaren alter Patienten; sie wird bei unseren Fällen nur zweimal vermißt. In den übrigen geprüften Muskeln der Bauchdecke und der oberen Extremität sind die Veränderungen zumeist geringer. Gleichartige Verbreiterungen der Basalmembran liegen in jüngeren Altersklassen bei jedem 3. Langzeitdiabetiker vor. Im Grundhäutchen werden Einschlüsse, in den Endothelzellen Lipofuscin und vermehrt Cytosome beobachtet. In der Skeletmuskulatur finden sich viel Lipopigment und selten große Myelinkörper oder Einschlüsse die globuläre Partikel bzw. filamentartige Gebilde erkennen lassen.
