Profil und Planung kieferchirurgischer Korrekturen

Zusammenfassung

Bei der kieferorthopädisch-kieferchirurgischen Behandlungsplanung müssen ästhetisch-kosmetische Faktoren berücksichtigt werden. Dazu muß einerseits der präoperative Profilverlauf objektiviert werden, andererseits wird es notwendig, auch das Behandlungsziel darzustellen und zu analysieren. Hierfür wird das in der Freiburger Klinik übliche System der Profilanalyse vorgestellt. Um postoperative Profilveränderungen vorhersagen zu können, wurden Nachuntersuchungen nach Progenieoperationen (Obwegeser/Dal Pont), nach Rückverlagerung des vorderen Oberkieferanteils und nach Kinnplastiken durchgeführt. Die Ergebnisse werden diskutiert und Folgerungen für die Therapie dargestellt.

Summary

When planning orthodontic-surgical treatment for adult patients with severe malocclusions, aesthetic factors must be taken into consideration. Therefore it is necessary to analyse the profile before the operation and to visualise the treatment objectives. In this paper we will introduce the profile analysis that we use in the Freiburg Clinic. We investigated post-operative changes of the profile in patients following mandibular set-back operations (Obwegeser/Dal Pont), anterior maxillary set-back and genioplasties. The consequences for subsequent treatment planning will be discussed in the light of these results.

La correction chirurgicale des malformations a des conséquences esthétiques non négligeables. Pour envisager un plan de traitement orthodontico-chirurgical, il faut une analyse de la téléradiographie de profil ainsi qu'une visualisation du résultat. Une méthode est proposée qui nous perment de choisir le meilleur type d'ostéotomie au point de vue du profil.
