Résumé
Le but du projet A du programme national de recherche No 1 est la prévention et la réduction des facteurs de risque des maladies cardio-vasculaires dans deux villes d'intervention en comparaison avec deux villes témoins, ainsi qu'une évaluation des méthodes préventives qui servira de base à des décisions politiques relatives à la promotion de la santé en Suisse. Le plan de l'étude prévoit une première phase d'examens de base suivie d'une période de trois ans pendant laquelle se feront les interventions préventives. On cherchera à obtenir des changements de comportement dans la population en général et dans certains groupes à risque. Ceci sera possible seulement par une participation active de la population à la planification et à l'application du programme. On s'efforcera de reccourir autant que possible aux ressources des villes d'intervention. Un comité local sera formé en tant qu'organe de décision. L'organisation d'un tel projet implique la recherche d'une formule qui d'une part donne le maximum de flexibilité à l'action locale mais d'autre part qui obéit aux nécessités de la standardisation et d'une évaluation rigoureuse.
Zusammenfassung
Das Ziel des Nationalen Forschungsprogra¯ms 1 ist die Verhütung und Reduktion der Risikofaktoren für kardiovaskuläre Krankheiten in zwei Interventionsgemeinden im Vergleich mit zwei Kontrollgemeinden sowie Evaluierung von präventiven Methoden als Grundlage für politische Entscheidungen bezüglich der Gestaltung der Prophylaxe in der Schweiz. Der Studienplan sieht eine Grunduntersuchungsphase vor, diese wird von einer drei Jahre dauernden Periode der Ein- un d Durchführung von präventiven Interventionen gefolgt. Dabei geht es um Verhaltensveränderungen der Gesamtbevölkerung wie auch gezielter Risikogruppen. Dies kann jedoch nur durch aktive Teilnahme der Bevölkerung an Planung und Durchführung des Programmes erreicht werden. Es geht also um die Mobilisierung sämtlicher Möglichkeiten der Gemeinden. Dazu wird ein lokales Aktionskomitee als eigentliches Entscheidungsorgan gebildet. Für die Organisation eines solchen Programmes muss deshalb diejenige Form gefunden werden, die einerseits maximale lokale Flexibilität in der Gestaltung der Aktionen erlaubt, andererseits aber den Bedürfnissen nach Standardisierung und rigoroser Evaluation Rechnung trägt.
Summary
The main objectives of the Swiss National Research Program on primary prevention of cardiovascular diseases are: 1) the reduction of known cardiovascular risk factors among the local populations of two intervention Communities compared to two “regular care” communities and 2) the provision of tested cost-effective methods for nation-wide use in the future control of CVD. The main focus during both the screening and the three years intervention phase will be on the total mobilization of community resources to induce behavior change (active participation of the population as well as the local health and social services system both in planning and implementation). A program such as this has to meet the challenge of developing a design that combines the needs for rigorous evaluation with a maximal amount of flexibility regarding the results of local initiative and involvement.
