Zusammenfassung
Bei 31 Patienten mit akutem Myokardinfarkt im Alter von 36–76 Jahren wurden arterielle Blutgase, pH und Standardbicarbonat bestimmt. Bei 18 von diesen 31 Patienten liegen die gleichen Meßwerte unter Atmung von reinem Sauerstoff vor. Zur Beurteilung hämodynamischer Veränderungen werden bei 16 von diesen letztgenannten 18 Patienten zusätzlich das HZV, blutige arterielle Drucke sowie der zentrale Venendruck gemessen. Die Untersuchung führte zu folgenden Ergebnissen:

Der arterielle O2-Druck war bei fast allen Patienten mit frischem Myokardinfarkt signifikant erniedrigt.

Die arterielle Hypoxämie muß auf eine Verteilungsstörung und eine erhöhte Beimischung venösen Blutes zurückgeführt werden, die ihrerseits Folgen der akuten Linksinsuffizienz mit Lungenstauung und Lungenödem sind.

Unter O2-Atmung steigen der periphere Widerstand im Systemkreislauf und der arterielle Druck an, während das Herzzeitvolumen signifikant abfällt. Eine gerichtete Änderung der Herzfrequenz tritt nicht auf.
Summary
31 patients with acute myocardial infarction aged 36 to 76 years were studied. In all patients arterial O2- and CO2-tensions as well as pH and standard bicarbonate were determined. In 18 of these patients the same measurements were carried out after breathing 99% oxygen. Cardiac output, arterial blood pressures, and central venous pressure were also studied. The following results were deduced:

Acute myocardial infarction is generally accompanied with arterial hypoxemia.

Arterial hypoxemia must be related to virtual and true venous shunting. Pulmonary congestion and edema is the primary cause of physiological shunting.

During oxygen-breathing systemic resistance and arterial pressure increase, while cardiac output and stroke volume tend to fall.
