Zusammenfassung
Nach einer kurzen historischen Einleitung werden von der ersten Beobachtung der verzögerten Blutgerinnung bei Hämophilie an, die Befunde beschrieben und analysiert, die von der normalen Gerinnung abweichen.
Die meisten früheren Untersuchungsresultate wurden auf Grund späterer Arbeiten widerlegt.
Die Annahme von dem Thromboplastinmangel der Gewebszellen Hämophiler erwies sich als unrichtig. Ebenso traf die Vermutung eines Defektes der Blutplättchen nicht zu, nachdem gezeigt wurde, daß der gerinnungsfördernde Inhalt derselben normal ist.
Nach Verlassen der cellulären Theorien folgte die wichtige Erkenntnis, daß der Gerinnungsdefekt plasmatisch bedingt ist. Leider wurde dieser irrtümlicherweise einem abnormalen Prothrombin zugeschrieben, was den weiteren Fortschritt der Forschung verzögerte. Prothrombin bei Hämophilie erwies sich später als normal. Es wurde jedoch eine verzögerte Umwandlung des Prothrombins in Thrombin festgestellt. Es findet eine unregelmäßige, stoßweise Bildung einer zur normalen Gerinnung unzureichenden Thrombinmenge statt.
Eine Gruppe von Untersuchern schrieb diese Tatsache dem Fehlen eines zum Aufbau des prothrombinumwandelnden Faktors, dem antihämophilen Globulin, zu. Eine andere Gruppe stellte für die verzögerte Thrombinbildung die Anwesenheit von Inhibitoren verantwortlich. Die Befunde beider Gruppen, die mit wechselndem Erfolg ihre Auffassungen stützten und verteidigten, wurden einer kritischen Betrachtung unterzogen. Schließlich wurde versucht, eine Reihe von Widersprüchen zu klären und zu überbrücken.
Summary
After a brief historical introduction, findings differing from normal clotting are described and analysed, beginning from the first observations of the delayed blood clotting in haemophilia.
Most of the early results were disproved on the basis of later investigations.
The assumption of thromboplastin deficiency in the tissue cells of haemophiliacs was found to be incorrect. The supposition of a defect of the platelets also didn't stand when it was demonstrated that their clot-promoting content was normal.
After the cellular theories were abandoned, the important realization followed that the clotting defect is a plasmatic condition. Unfortunately, this clotting defect was erroneously ascribed to an abnormal prothrombin which delayed further progress of research. Later, prothrombin proved to be normal in haemophilia. However, a delayed conversion of prothrombin to thrombin was established. An irregular stepwise formation of thrombin takes place in adequate amounts for normal clotting.
One group of investigators ascribed this fact to the absence of a factor, the antihaemophilic globulin, necessary for the formation of the prothrombin-converting principle. Another group held the presence of inhibitors responsible for the delayed formation of thrombin. The findings of the representatives of both groups who presented and defended their views with varying success were given critical consideration. Finally, an attempt was made to clarify and reconcile a number of contradictory views.
