Conclusions
One can assume that, as the first stadium of a hepatogen diabetes, a hyperglycemia and an increase of the free fatty acids in the serum turns up with regularity. This is caused by the alteration of the liver cell function and of the circulation in the liver in the beginning of a cirrhosis.
The hyperglycemia leads, according to the presence or the absence of a simultaneous damage of the pancreas, to a moderate or heavy hypertrophy and hyperplasia of the cells of islets. The hyperglycemia and, therefore, the chronic hyperstimulation is maintained by insulin antagonising factors. It is warranted to consider the inhibition of the insulin effect on the glucose oxidation in the musculature by the increase of the free fatty acids in the serum which already appears in the initial stadium. The permanent overstress of the B-cell-system results finally in an exhaustion and overt diabetes mellitus.
The pathogenetic operative mechanisms mentioned in the introduction can, therefore, not be considered to be alternatives in the pathogenesis of the hepatogen diabetes, with the exception of sclerosis of pancreas. They operate, in any case, temporal and causal dependent from each other.
