Zusammenfassung
Der erste Teil einer Serie von Untersuchungen mit dem Ziel einer Aufklärung der appetit- und gewichtsteigernden Wirkung von Cyproheptadin = 1-Methyl-4-(5-dibenzo-[a,e]-cycloheptatrienyliden)-piperidinhydrochlorid umfaßt klinische Erfahrungen und Hormonjodstudien. Folgende Ergebnisse werden erzielt:

Bei untergewichtigen und normalgewichtigen ambulanten Patienten beiderlei Geschlechts im Alter zwischen 19 und 65 Jahren führt tägliche Zufuhr von 4mal 4 mg Cyproheptadin zu einer Steigerung des Appetits, z.T. unter Angabe von Heißhunger oder spezifischem Appetit auf süße Speisen. Niemals entsteht Freßlust. Vielmehr verkürzen sich die Zeiten zwischen den einzelnen Mahlzeiten. Die Appetitsteigerung ist unabhängig von Alter, Allgemeinzustand oder Genese einer möglicherweise vorhandenen Inappetenz.

Es besteht ein enger zeitlicher Zusammenhang zwischen Einnahme von Cyproheptadin, Appetitsteigerung und Gewichtsansatz. Die innerhalb von 14–21 Tagen auftretende Gewichtszunahme beträgt durchschnittlich 1,54 kg. Dabei entwickelt sich das Bild einer Übergewichtigkeit durch vermehrte Nahrungszufuhr.

Der Mechanismus der appetit- und gewichtsteigernden Wirkung von Cyproheptadin ist unabhängig von Veränderungen der Schilddrüsenfunktion. Auch nach mehrmonatiger Anwendung des Mittels entwickeln sich keine hypothyreoten Züge. Bei wiederholten Bestimmungen weisen die Serumkonzentrationen von PBI127 und BEI127 sowie die Serumaktivität von Exophthalmus-produzierendem Faktor (EPF) keine signifikanten Änderungen auf. Frühestens 14 Tage nach Behandlungsbeginn nehmen die Funktionswerte im T3-Test leicht, aber signifikant ab. Die Befunde werden allgemein und speziell im Hinblick auf die Fragestellung erörtert.
Summary
The first part of a series of studies aiming at clarifying the appetite-stimulating and weight-gain effect of Cyproheptadine (= 1-methyl-4-(-dibenzo-[a,e]-cycloheptatrienylidene)-piperidinehydrochloride) comprises clinical experience and hormone-iodine-studies. Results are as follows:
1.In underweight and normalweight out-patients of both sexes between the ages of 19 and 65 years, a dose of 4 mg of Cyproheptadine q.i.d. leads to an increase of appetite — in some cases accompanied by hyperorexia or a specific craving for sweets. Phagomania was not observed. The intervals between meals become shorter. The appetite stimulation occurred independent of age, general condition or origin of a probably present anorexia.
2. There is a close timely relation between the application of Cyptoheptadine, the onset of appetite stimulation and weight gain. We observed an average elevation of body weight of 1,54 kgs within 14 to 21 days, impressing as overweight due to increased food intake. 3.The mechanism of action of the appetite stimulating and weight gain effect of Cyproheptadine is independent of changes of the thyroid function. Even after several months of medication, no hypothyroid symptoms occurred. At repeated determinations, the concentrations of PBI127 and BEI127 in the serum as well as the serum activity of exophthalmus producing factor (EPF) did not show significant changes. 14 days after initiation of therapy, at the earliest, T3-test values decreased slightly but significantly. The findings are discussed in general and specifically with regard to the questions raised.
