Summary
Information was collected concerning 1400 schizophrenic patients who had been continuously resident in six hospitals in Lower Saxony for more than five years on 15. 10. 1964. Sex, marital status, separation from home, education, vocational training, number of admissions and age of onset of the illness were considered. The combination of some of these characteristics and comparable results in the literature are discussed. Fundamental characteristics of length of hospital stay and methodological differences from studies of the course of schizophrenia are described.
Sex does not seem to affect length of stay in spite of considerable age differences between males and females. Single patients are more likely to suffer from schizophrenia, are more often admitted and remain in hospital longer, than married. A poor education or lack of vocational skills leads to a long stay while above average endowment delays it. In the higher age-group, connected social and medical factors make a long stay more likely. Separation from home was a particularly important factor, with a high representation in the series. The poor social status of those living away from home contributed to a long stay.
Finally, it has been demonstrated that, without an improvement in psychiatric administration and social treatments, the numbers of long-stay patients are likely to increase.
Résumé
L'auteur examine les variables suivantes: sexe, situation familiale, émigration, formation scolaire et professionnelle, nombre d'hospitalisations, âge au début de la maladie, à la première admission et à l'admission décisive ainsi qu'à une journée déterminée, chez presque 1400 schizophrènes qui, le 15. 10. 1964, étaient hospitalisés depuis 5 ans sans interruption dans 6 cliniques psychiatriques de Basse-Saxe. Les combinaisons de ces variables et des résultats comparables dans la littérature sont discutés. Les conditions fondamentales d'une hospitalisation durable ainsi que les différences méthodologiques dans les études sur l'évolution de la schizophrénie sont discutées.
Une influence du sexe sur la durée de l'hospitalisation ne put pas être mise en évidence malgré des différences d'âge considérables entre hommes et femmes. En revanche, la situation familiale joue un rôle important: les personnes célibataires sont plus souvent affectées de schizophrénie, sont plus fréquemment hospitalisées et l'hospitalisation est plus souvent durable que chez les personnes mariées. Une mauvaise formation scolaire ou professionnelle favorisait et accélérait une hospitalisation durable, tandis qu'une formation supérieure la retardait. Des facteurs sociaux et découlant de la maladie liés à un âge plus avancé favorisaient une hospitalisation durable. Parmi les facteurs influençant la composition du matériel examiné, l'émigration était particulièrement importante. Les personnes tombées malades avant l'émigration étaient nettement sous-représentées, tandis que celles qui tombèrent malades après l'émigration étaient fortement surreprésentées; la situation sociale défavorable des émigrés favorisait une hospitalisation durable. Pour terminer, l'auteur souligne que sans une amélioration essentielle des institutions et des possibilités de traitement psychiatrique, il faudra craindre une augmentation des hospitalisations prolongées de schizophrènes.
Zusammenfassung
Es wird über die Merkmale Geschlecht, Familienstand, Vertreibung, Schul- und Berufsbildung, Zahl der Krankenhausaufenthalte, Alter bei Krankheitsbeginn, bei der ersten und bei der entscheidenden Aufnahme und an einem Stichtag bei fast 1400 Schizophrenen berichtet, die am 15. 10. 1964 fünf Jahre ohne Unterbrechung in stationärer Behandlung von 6 niedersächsischen Landeskrankenhäusern standen. Die Kombinationen einzelner dieser Merkmale miteinander und vergleichbare Resultate in der Literatur werden erörtert. Grundsätzliche Bedingungen einer Dauerhospitalisierung und methodologische Unterschiede von Studien über den Verlauf einer Schizophrenie werden besprochen.
Ein Einfluß des Geschlechtes auf die Dauerhospitalisierung ließ sich trotz beträchtlicher Altersunterschiede zwischen Männern und Frauen nicht nachweisen. Dagegen spielt der Familienstand eine wichtige Rolle: ledige Personen erkranken öfter an Schizophrenie, werden öfter deswegen hospitalisiert und auch dauerhospitalisiert als verheiratete. Eine schlechte Schul- und Berufsbildung förderte und beschleunigte die Dauerunterbringung, eine überdurchschnittliche zögerte sie hinaus. Mit einem höheren Lebensalter verbundene soziale und krankheitsbedingte Faktoren förderten eine Dauerunterbringung. Unter den Faktoren, die die Zusammensetzung des untersuchten Krankengutes wesentlich beeinflußten, war die Heimatvertreibung besonders wichtig: vor der Vertreibung erkrankte Personen waren deutlich unter- und nach der Vertreibung erkrankte stark überrepräsentiert; die schlechtere soziale Lage der Vertriebenen unterstützte eine Dauerunterbringung.
Abschließend wird darauf hingewiesen, daß es ohne eine gründliche Verbesserung der psychiatrischen Einrichtungen und Behandlungsmöglichkeiten zu einer Zunahme langjährig untergebrachter Schizophrener kommen wird.
