Summary
This paper describes the characteristics of girls referred to a consulting psychiatrist by the Young Mothers Clinic, part of a comprehensive service program for school-age mothers in New Haven, Conn. This program provided obstetrical, educational, and social services to the study group, which consisted of 180 patients who entered the clinic over a two year period. All of the study group were under 18 years of age and unwed when they registered for the clinic, and most were nonwhite. — Twenty-five of the 180 girls were referred to the psychiatrist. Common reasons for referral included lack of attention, delinquency, learning difficulties at school, withdrawn behavior, depression, suicidal threats, and chaotic home situations. Those referred were found frequently to have 1. a history of contact with juvenile court and/or 2. low scores on the Wechsler intelligence scales and poor scores on the Bender Gestalt test. — White girls were more likely to be referred than were nonwhite girls. Girls 15 years of age or under were more likely to be referred than older girls. These two groups (i.e. the younger group and the white group) tended to come from extremely disorganized homes. — Clinically, the psychiatrist was impressed by the repeated history of maternal deprivation among the referred patients. She hypothesized that the girls' unfulfilled yearning for closeness with their mothers may have driven them toward early physical relationships with boys, resulting in the pregnancies. Therapeutic recommendations were difficult to implement primarily because the girls, for various reasons, did not follow through with referrals to other sources of care.

Ce travail décrit les caractéristiques de jeunes filles adressées à un psychiatre consultant par la clinique des jeunes mères, qui fait partie d'un ensemble complet de services pour mères en âge scolaire à New Haven, Conn. Cet ensemble disposait de services obstétricaux, éducatifs et sociaux pour le groupe soumis à l'étude, groupe qui comprenait 180 patientes admises à la clinique sur une période de 2 ans. Toutes les patientes de ce groupe avaient moins de 18 ans et étaient célibataires au moment de l'admission, et la plupart n'étaient pas des blanches. — 25 des 180 jeunes filles ont été adressées au psychiatre. Les principales raisons en étaient: manque d'attention, délinquance, difficultés scolaires, comportement de repli sur soi-même, dépression, menaces de suicide et situation familiale chaotique. Parmi ces jeunes filles, on a trouvé fréquemment: 1. un contact antérieur avec un tribunal pour mineurs et/ou 2. de mauvais résultats aux échelles d'intelligence de Wechsler et des résultats médiocres au test de Bender (Gestalt test). — Les jeunes filles blanches étaient plus fréquemment adressées au psychiatre que les jeunes filles non-blanches, les jeunes filles de 15 ans ou moins plus souvent que les jeunes filles plus âgées. Ces deux groupes (c'est-à-dire le groupe des plus jeunes et le groupe des blanches) tendaient à provenir de foyers extrêmement perturbés. — Du point de vue clinique, le psychiatre a été frappé par la fréquence de privation maternelle parmi les patientes qui lui étaient adressées. Il en a tiré l'hypothèse que le besoin non satisfait de ces jeunes filles d'un contact avec leur mère peut les avoir amenées à entretenir des relations physiques précoces avec des garçons, relations aboutissant à la grossesse. Des indications thérapeutiques se sont avérées difficiles à appliquer prinicpalement parce que les jeunes filles, pour diverses raisons, négligeaient de les poursuivre par d'autres consultations.
Zusammenfassung
In dieser Arbeit werden die charakteristischen Merkmale von Mädchen beschrieben, die von der Klinik für junge Mütter an einen beratenden Psychiater überwiesen wurden. Die Klinik ist in New Haven, Conn. Bestandteil eines umfassenden Versorgungsprogramms für Mütter im Schulalter. Dieses Programm bot einer Untersuchungsgruppe geburtshelferische, pädagogische und soziale Dienste an. Die Gruppe bestand aus 180 Patienten, die die Klinik während einer 2-Jahresperiode aufgesucht hatten. Alle waren unter 18 Jahren alt und ledig, als sie sich in die Behandlung der Klinik begaben, und die meisten waren Farbige. — Fünfundzwanzig von den 180 wurden einem Psychiater überwiesen. Die üblichen Überweisungsgründe umfaßten Vernachlässigung, Delinquenz, Lernschwierigkeiten in der Schule, soziale Zurückgezogenheit, Depression, Selbstmordgefährdung und chaotische häusliche Verhältnisse. Bei den Überwiesenen fand man häufig 1. vergangene Kontakte mit dem Jugendgericht und/oder 2. niedrige Punktzahlen auf den Wechsler-Intelligenzskalen und schlechte Punktzahlen im Bender-Gestalttest. — Weiße Mädchen wurden eher überwiesen als farbige. Mädchen mit 15 Jahren oder darunter wurden eher überwiesen als ältere Mädchen. Diese beiden Gruppen (d.h. die jüngere und die weiße Gruppe) kamen oft aus extrem ungeordneten häuslichen Verhältnissen. — Aus klinischer Sicht war es beeindruckend, daß in wiederholten Fällen überwiesene Patientinnen in der Vergangenheit ihre Mutter verloren hatten. Die Psychiatrin vermutete, daß die unerfüllte Sehnsucht der Mädchen nach mütterlicher Nähe sie zu frühen körperlichen Beziehungen mit Jungen getrieben haben kann, die zu Schwangerschaften führten. Therapeutische Empfehlungen waren schwer durchführbar, weil die Mädchen aus verschiedenen Gründen nicht allen weiteren Überweisungen an andere Versorgungsdienste folgten.
