Ein Mehrzweckinstrument zur Entfernung des Mondbeines bei schwerer Lunatummalacie und veralteten Handwurzelverletzungen

Summary

The so called lunate seizing forceps is presented. It has been constructed to facilitate the excochleation of the lunate bone especially in the Graner's operation and the Steinhäuser's plastic. The instrument can be used to push away and to loosen the ligament-and capsular-connections attached to the lunate bone with the most possible indulgence of the neighbouring cartilage parts. Especially in the progressive states of Kienböck's disease with softening and fragmentation of the lunate bone the tool helps to avoid the falling to pieces during the act of taking the bone out. Consequently it enables the operator to extirpate the lunate bone entirely, in a careful way and in a single act. Moreover the forceps is qualified excellently as a holding-forceps for fixing half-tube-plates to small tubular bones during the operation, for the exact adaptation of fragments, for the extraction of bone sequesters or free bodies in joints and similar operations. So it can be used as a multi-purpose instrument.

L'auteur présente une pince spéciale à saisir l'os lunatum. Elle a été construite préponderantement pour faciliter à l'opérateur l'extirpation de l'os lunatum pendant les opérations plastiques de Graner et de Steinhäuser. L'instrument fait possible la séparation de l'os lunatum de ses jonctions capsulaires et ligamentaires très fixes avec le plus grand soin aux cartilages voisins; d'autre part elle évite — spécialement dans les phases avancées de la maladie de Kienböck, dans lesquelles le lunatum généralement est mollit et fragmenté, — la destruction complète artificielle pendant l'extraction. Ainsi l'instrument fait possible d'extirper l'os lunatum en totalité, avec le plus grand ménagement et par une seule action. De plus la pince peut-être utilisée très bien par example comme pince à soutenir pendant l'application d'un plat demitube à l'os tubiforme petit pour l'ostéosynthèse, pour l'adaptation exacte de fragments ou pour l'extraction de séquestres d'os et des opérations analogues. Ca veut dire que — à part de la maladie de Kienböck — il y a beaucoup de possibilités pour utiliser cet instrument universel.

Zusammenfassung

Es wird die sogenannte Lunatum-Faßzange vorgestellt. Sie wurde konstruiert, um dem Operateur die vorwiegend bei der Graner-Operation und bei der Steinhäuser-Plastik notwendige Excochleation des Os Lunatum zu erleichtern. Das Instrument dient einmal dem Abschieben und Lösen der am Mondbein fest haftenden Band-und Kapselverbindungen unter bestmöglicher Schonung der benachbarten Knorpelflächen. Zum anderen verhindert das Gerät — vor allem in den Spätstadien der Kienböckschen Erkrankung mit weitgehender Erweichung und Fragmentation des Mondbeines — den Zerfall desselben bei der Herausnahme. Es ermöglicht somit dem Operateur die Exstirpation des Os Lunatum in seiner Gesamtheit, auf schonende Weise und in einem Zuge. Darüber hinaus eignet sich die Zange in hervorragender Weise als Haltezange beim Anbringen von Halbrohrplatten an kleinen Röhrenknochen, zum exakten Adaptieren von Fragmenten, zur Extraktion von Knochensequestern, freien Gelenkkörpern und ähnlichem und beweist damit ihre vielseitige Verwendungsmöglichkeit.
