Abstract

The maintenance of a stable internal milieu by the kidney involves an intricate array of functions, and calls into play a variety of mechanisms including hydraulic forces, selective permeability barriers, specialized transport systems and exactingly regulated synthetic machinery. Replacement of renal function artificially is a formidable task in view of the need to balance intended goals and available technologies. The elegant apparent simplicity of the hemodialysis concept contrasts sharply with the acknowledged complexity of renal mechanisms. It is a tribute to the imaginative approach of hemodialysis pioneers to attempt a formidable task with simple means. With time, however, the ostensible simplicity of dialysis has been shown to mask cardinal sophistication and an underlying complexity.
