Abstract
Een paniekstoornis wordt gekenmerkt door het herhaaldelijk optreden van onverwachte paniekaanvallen en een langere periode van angst, onrust of ongemak ten gevolge hiervan. Een paniekaanval is een korte periode (minuten) van hevige angst of onbehagen die gepaard gaat met een verscheidenheid aan symptomen, zoals hartkloppingen, transpireren of een gevoel van ademnood. Deze aanvallen worden door de patiënt als catastrofaal beleefd. De patiënt vreest bijvoorbeeld dat hij of zij een hartaanval krijgt, gek wordt of de controle verliest en zich ten overstaan van anderen belachelijk maakt. Vaak ook is er de angst dat ontsnappen uit de ruimte waarin de patiënt zich bevindt, niet mogelijk is op het moment dat de patiënt zich weer ‘raar’ voelt worden. Een paniekstoornis gaat daarom vaak gepaard met vermijdingsgedrag, agorafobie genoemd. Naast karakteristieke situaties die vermeden worden, zoals mensenmassa's, winkels, openbare ruimtes, openbaar vervoer, autorijden, bruggen en tunnels, zijn er ook minder in het oog springende situaties en activiteiten die vermeden worden, zoals lichamelijke inspanning (sport, opwinding), scherp voedsel of koffie. Ook meer subtiel vermijdingsgedrag komt voor, bijvoorbeeld in de buurt van een gangpad gaan zitten of de deuren naar buiten in de gaten houden. Hoewel volgens de DSM-IV (APA, 1994) een diagnose paniekstoornis zonder agorafobie gesteld kan worden, blijkt in de praktijk bijna altijd enige vorm van vermijding aanwezig te zijn. De life time prevalentie van de paniekstoornis ligt tussen 1,5% en 3,5% (APA, 1994). Hiermee is het de meest voorkomende angststoornis.
