Abstract
This paper will focus on the operative management of patients with giant aneurysms of the carotid and middle cerebral arteries. These comprise approximately 70% of the giant intracranial aneurysms upon which the author has operated. The carotid sites include ophthalmic (Ophth), posterior communicating (PCA), and anterior choroidal (ACh) arteries, and the internal carotid artery bifurcation (ICAB). These cases occurred in the author’s personal experience with over 700 aneurysms. This data and the location of the operated aneurysms is summarized in Tables 1 and 2. Although there are many similarities in the management of all patients with subarachnoid hemorrhage (SAH) from aneurysms, specific details of these locations will be stressed.
