Zusammenfassung
Anfang der siebziger Jahre ist das Podophyllotoxin VP 16-213 (Etoposid) in klinische Studien eingeführt worden. Die Schweizerische Arbeitsgruppe für klinische Krebsforschung (SAKK) hat sich früh mit den Wirkungen dieser Substanz auseinandergesetzt. Die Abhängigkeit der antitumoralen Wirksamkeit vom Verabreichungsschema („schedule dependency“ wurde 1978 von Cavalli untersucht (1). Patienten mit kleinzelligem Bronchuskarzinom wurden in 3 Gruppen unterteilt. Die Gruppe A erhielt 250 mg/m2 Etoposid intravenös einmal pro Woche; die Gruppe B 500 mg/m2 peroral, auf 3 Tage verteilt einmal pro Woche; die Gruppe C erhielt 850 mg/m2 peroral, auf 5 Tage verteilt alle 3 Wochen. Der Therapieerfolg wurde nach 6 Wochen gemessen. Eine objektive Tumorrückbildung wurde bei 20% (4/20) der Gruppe A, bei 65% (11/17) der Gruppe B und bei 42% (8/19) der Gruppe C registriert. Die Remissionsrate der Gruppe B war statistisch signifikant besser, verglichen mit der Gruppe A. Obwohl die hämatologische Toxizität für alle 3 Behandlungsgruppen vergleichbar war, erhielten die Patienten in der Gruppe B während der 6 Behandlungswochen eine höhere Gesamtdosis von Etoposid (1280, 2480 und 1890 mg/m2 für die Gruppen A, B und C).
